The German NOTES registry (GNR) was initiated by the German Society for General and Visceral Surgery (DGAV) as a treatment and outcome database for natural orifice transluminal endoscopic surgery (NOTES). ⋯ The German surgical community ensures a safe and responsible introduction of the new NOTES operation techniques with its active participation in the GNR. Despite an overall low complication rate, the high number of procedures in the GNR permitted the identification of method-specific complications. This knowledge can be used to further increase the safety of NOTES in practice.

The development of a postoperative respiratory insufficiency is typically caused by several factors and include patient-related risks, the extent of the procedure and postoperative complications. Morbidity and mortality rates in acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are high. It is important to have consistent strategies for prevention and preoperative conditioning is essential primarily for high-risk patients. ⋯ In critical cases an extracorporeal lung assist is considered to be beneficial as a bridge to recovery and for realizing a protective ventilation protocol. Different systems with separate indications are available. The temporary application of a lung assist allows thoracic surgery to be performed safely in patients presenting with insufficient respiratory function.

Many patients undergoing thoracic surgery are treated with anticoagulants or platelet aggregation inhibitors because of different comorbidities. Additionally the risk of thromboembolic events is increased perioperatively; therefore, a perioperative thrombosis prophylaxis is necessary. On the other hand the risk of intraoperative or postoperative bleeding is increased because of the volume of the thoracic cavity and the partially negative intrathoracic pressure. ⋯ For the correct perioperative application of vitamin K antagonists, platelet aggregation inhibitors, new oral anticoagulants and drugs for thrombosis prophylaxis, it is of major importance to conceive an individual risk profile of thoracic surgery patients. However, there are not enough evidence-based data referring to the correct perioperative management for new oral anticoagulants.

Minimally invasive procedures have increasingly been used to treat pilonidal disease; however, the mid-term and long-term results have not been evaluated extensively yet. ⋯ Approximately three quarters of selected patients with pilonidal disease benefit from minimally invasive "pit picking" surgery.
