Venovenous hemodiafiltration and hypothermia for treatment of cerebral edema associated with hyperammonemia.
We report a 50 hour old newborn with inborn urea cycle disorder and hyperammonia of 2320 umol/L. The pharmacological treatment of the first metabolic crisis was combined with venovenous hemodiafiltration and therapeutic hypothermia to rescue the patient from a life-threatening cerebral edema.

Clinical Pulmonary Infection Score to diagnose ventilator-associated pneumonia in children.
There is a need to validate and suggest easy clinical method for diagnosis of ventilator-associated pneumonia (VAP) in developing countries. ⋯ Simplified CPIS is useful in patients on mechanical ventilation to diagnose ventilator-associated pneumonia.

To analyze cardiovascular parameters by echocardiography in preterm infants with patent ductus arteriosus (PDA). ⋯ In preterm infants with hsPDA, there was a volume load of the left heart causing increased stroke volume and cardiac output. The hsPDA could be detected by echocardiography even in the first 48 hours. The left atrial volume index may be a better indicator of the volume load of the heart.

Rituximab is a chimeric mouse-human monoclonal antibody against the CD 20 antigen on the surface of B lymphocytes. It binds to CD20 and causes B cell death by antibody dependant cell-mediated cytotoxicity, complement mediated cytotoxicity and apoptosis. It leads to rapid and sustained depletion of B cells. ⋯ Nephrotic syndrome and opsoclonus-myoclonus syndrome are among the emerging indications for rituximab. In solid organ transplantation, rituximab is useful in the prevention and treatment of acute and chronic rejection as well as in post transplantation lymphoproliferative disease. Toxicity includes acute infusion reactions, susceptibility to bacterial infections, and reactivation of viral infections.
