ReviewAntinuclear antibodies in autoimmune disease. Significance and pathogenicity.
A large number of antigen-antibody systems have been described in association with connective tissue diseases. However, with the exception of antibodies to dsDNA, none of them have yet been successfully implicated in the pathogenesis of autoimmune diseases. It is also unclear why specific ANA are associated just with certain diseases, for example, anti-Sm with systemic lupus erythematosus. Although many questions remain about what triggers ANA production and whether these antibodies are innocent bystanders or disease inducers or enhancers, ANA serology can still be very useful to the clinician.

Bacterial meningitis is an acute and serious illness associated with significant morbidity and mortality. Although methods to make the diagnosis are relatively simple and effective antimicrobial agents are available, bacterial meningitis accounts for more than 2000 reported deaths per year in the United States. The majority of cases and deaths occur in otherwise healthy individuals.

The results of CT brain scan in diagnosing and monitoring the course of brain abscesses and the recognition of the major role that anaerobic bacteria play in causing this disease have led to a reduction in disease mortality and have allowed a greater percentage of infected patients to recover without major neurosurgical procedures. Monitoring intracranial pressure, the availability of new antibiotics, and recognition and treatment of some of the more unusual syndromes associated with defective or suppressed immune systems have helped lead to a reduction in disease mortality from about 50 per cent to 10 per cent with excellent functional recovery in the majority of the patients.

ReviewThe low back pain syndrome. Diagnostic impact of high-resolution computed tomography.
This article focuses on low back pain which is directly related to the spinal axis and its supporting structures (spondylogenic back pain). This type of back pain is particularly prominent in our society, is frequently managed surgically, and has benefited from most advanced diagnostic imaging with computed tomography.

The investigational antiarrhythmic agents available for use in this country are predominantly class I drugs with local anesthetic membrane effects. These drugs are often used successfully to control arrhythmias refractory to treatment with the standard antiarrhythmic drugs. Side effects often limit their use, and particular attention needs to be paid to their cardiac side effects, such as exacerbation of arrhythmia or enhanced conduction defects.
