There is growing interest in the relationship between allergies and autoimmune diseases, although previous studies have yielded inconsistent results. The thyroglobulin (Tg)/thyroid peroxidase antibody (TPOAb) group consisted of 217 patients with positive thyroglobulin antibody (TgAb) and/or TPOAb test results. Another set of 217 age- and sex-matched individuals with both TgAb- and TPOAb-negative results were selected as control group. ⋯ TgAb positivity (OR, 2.333) was identified as a risk factor for AR, AD, or CSU in Chinese patients, suggesting the involvement of thyroid autoantibodies in the pathogenesis of atopic reactions. Multivariate regression analysis also confirmed that the presence of TgAb (P = .004), rather than TPOAb (P = .468), had a significant impact on the occurrence of allergic disease. Physicians should carefully monitor atopic symptoms in individuals with elevated TgAb or TPOAb levels to reduce the risk of allergic diseases, such as AR, AD, and CSU.

Usefulness of bispectral index and patient state index during sevoflurane anesthesia in children: A prospective observational study.
Cortical electroencephalography-based devices are used to monitor the depth of anesthesia. In this study, we evaluated the values of bispectral index (BIS) and patient state index (PSI) during sevoflurane anesthesia in children. The ability/accuracy of BIS and PSI to predict the maintenance and recovery state of anesthesia was evaluated based on prediction probability (Pk) values and the secondary outcomes were agreement and correlation of 2 monitors. ⋯ Pk values were comparable for BIS and PSI. Agreement between BIS and PSI measurements in the same state was relatively good. Therefore, these monitors are appropriate for monitoring for different state of anesthesia in pediatric population.

Ectopic varices are the collateral circulation of portal vein located anywhere in the gastrointestinal tract other than the esophageal and gastric regions. Rupture of these varices often results in life-threatening hemorrhage. Management guidelines for ectopic variceal bleeds are not yet standardized because cases are rare and treatment approaches described in the literature vary considerably. ⋯ Ectopic varices, which are rare sequelae of portal hypertension, need to be taken seriously because bleeding from these varices can be catastrophic. We report a case of isolated sigmoid variceal rupture and hemorrhage due to portal hypertension in cirrhosis. The patient experienced failure of endoscopic hemostasis and sigmoid colon venous coil embolization. She was eventually successfully brought to hemostasis by surgery.

COVID-19 and Guillain-Barré syndrome: A single-center prospective case series with a 1-year follow-up.
Single reports of Guillain-Barré syndrome (GBS) have been reported worldwide during the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic. While case reports are likely to be biased toward uncommon clinical presentations, systematic assessment of prospective series can highlight the true clinical features and spectrum. In this prospective, observational study, we included all consecutive patients who developed GBS. ⋯ Cerebrospinal fluid search for SARS-CoV-2 RNA and antiganglioside antibodies were negative in all COVID+ patients. Temporal clustering of cases, coinciding with the waves of the pandemic, and concomitant reduction of the incidence of COVID-negative GBSs may indicate a role for SARS-CoV-2 infection in the development of GBS, although the association may simply be related to a bystander effect of systemic inflammation; lack of prevalence of specific GBS subtypes in post-COVID-GBS also support this view. GBS features and prognosis are not substantially different compared to non-COVID-GBS.

Individuals on immunosuppressive therapies experience greater morbidity and mortality due to vaccine-preventable illnesses, but there are low rates of adherence to immunization guidelines within this population. ⋯ The intervention successfully addressed vaccine hesitancy and improved influenza immunization rates in an immunosuppressed population receiving care from a specialty clinic. Implementing a similar model across specialty clinics may improve vaccination rates for influenza, coronavirus disease 2019, and other vaccine-preventable illnesses in other populations.
