Oocyte matched follicular fluid oxidant, antioxidant status, and pro- and anti-inflammatory cytokine levels were assessed to reveal a possible effect of local-intrafollicular levels of these markers on the individual oocyte with its quality, ability to achieve fertilization, further embryo development, and pregnancy. A cross-sectional study of infertile women with diminished ovarian reserve undergoing antagonist protocol in vitro fertilization (IVF); in the form of ICSI, and fresh single embryo transfer were included. When follicular fluid was collected, each ovarian follicle was aspirated independently, and each follicular fluid was collected into a separate test tube to match it with a single cumulus-oocyte complex obtained from the same follicle. ⋯ Follicular fluid IL-8 value was significantly low with positive pregnancy results (P < .001). Serum oxidative stress status and follicular fluid pro-inflammatory cytokines were associated with IVF outcomes. This unique study might guide IVF practice with the aim of developing and establishing more effective therapeutic strategies and choosing embryos with more potential for success.

Pathological pseudoprogression to anti-PD-1 inhibitor in metastatic periampullary carcinoma: Case report.
Pseudoprogression has been deemed as a rare clinical phenomenon during the treatment of immune checkpoint inhibitors in patients with advanced cancers, especially in periampullary carcinoma, however, leaving potential molecular mechanism remain unknown. ⋯ Results of the present case suggested that pseudoprogression, along with hair and mustache repigmentation, possibly caused by anti-PD-1 inhibitors, may also happen in patients with periampullary carcinoma, which should be paid attention to. The potential mechanism should be further investigated.

Dementia is a progressive disease that worsens over time as cognitive abilities deteriorate. Effective preventive interventions require early detection. However, there are no reports in the literature concerning apps that have been developed and designed to predict patient dementia classes (DCs). This study aimed to develop an app that could predict DC automatically and accurately for patients responding to the clinical dementia rating (CDR) instrument. ⋯ On the basis of a combination strategy and a decision rule, a 74-item ANN model with 285 estimated parameters was developed and included. The development of an app that will assist clinicians in predicting DC in clinical settings is required in the near future.

Left atrial (LA) remodeling is closely related to the occurrence of cerebral stroke; however, the relationship between early-stage impaired deformability of the left atrium and stroke/transient ischemic attack (TIA) remains unclear. The aim of this study was to evaluate the changes in LA deformability and to assess its relationship with stroke/TIA events using speckle tracking echocardiography. A total of 365 patients with paroxysmal atrial fibrillation (non-stroke/TIA [n = 318]; stroke/TIA [n = 47]) underwent comprehensive echocardiography with speckle tracking imaging to calculate mean LA longitudinal strain and strain rate values from apical 4-chamber, 2-chamber, and 3-chamber views. ⋯ The regression equation was y = -1.430-0.394X1 + 0.012X2 + 0.019X3 (P < .001) (y, SRE; X1, E/A ratio; X2, LA VI; X3, body mass index). In multivariate logistic regression analyses, SRE and sex ratio were independent risk factors for stroke/TIA (SRE, odds ratio 2.945 [95% confidence interval 1.092-7.943]; P = .033; sex, odds ratio 0.462 [95% confidence interval 0.230-0.930]; P = .031). Among patients with paroxysmal atrial fibrillation, SRE reflected impaired deformability of the left atrium in the early stages and was associated with the risk of stroke/TIA.

De novo non-alcoholic fatty liver disease (NAFLD) after pancreatectomy is a recognized phenomenon; however, its pathophysiology is poorly understood. This study aimed to determine the incidence and identify peri-operative risk factors for the development of de novo NAFLD within various pancreatectomy groups. This single-center retrospective cohort study included patients who underwent pancreatectomy between 2000 and 2020. ⋯ Time to development of hepatic steatosis in the malignant group was 26.4 months and was significantly shorter by an average of 6 months when compared to the benign group (32.8 months, P = .03). Higher pre-operative body mass index was associated with new-onset NAFLD (P = .03). Pre-operative body mass index is a significant predictor for de novo NAFLD and highlights a group that should be closely monitored post-operatively, especially after resections for pancreatic malignancy.
