ChatGPT performance in the medical specialty exam: An observational study.
In our study, we aimed to evaluate the success of ChatGPT by determining its performance in the last 5 medical specialty exams (MSE) conducted and its ranking among the candidates of that year, and to determine its potential use in healthcare services. Publicly available MSE questions and answer keys from the last 5 years were scanned, a total of 1177 questions were included in the study, all questions were asked to the ChatGPT (OpenAI; San Francisco, CA) GPT-3.5 series, which is the March 23, 2023 version. The average score and rank that ChatGPT would receive if it had entered the exam that year were determined. ⋯ ChatGPT statistically significantly answered a higher proportion of questions correctly in the short questions group compared to the long questions group (P = .03), and in the single select multiple choice questions group compared to the multi-select multiple choice questions group (P < .001). ChatGPT has been successful in the MSE, a challenging exam for doctors in our country. However, it is a fact that ChatGPT is still behind the expert in the field for now, and what will happen with program developments in the future is a matter of curiosity for all of us.

Refractory cardiac arrest caused by type I Kounis syndrome treated with adrenaline and nicorandil: A case report.
Kounis syndrome is a rare but life-threatening anaphylactic reaction that can lead to acute coronary syndrome and cardiac arrest, and requires prompt diagnosis. Adrenaline, which is used to treat anaphylaxis, may cause coronary vasoconstriction and worsen ischemia, whereas coronary vasodilators may dilate systemic vessels and exacerbate hypotension. Delayed diagnosis of Kounis syndrome and inadequate therapeutic intervention may thus lead to a poor outcome. ⋯ If anaphylaxis leads to refractory shock and cardiac arrest, ischemic changes on the electrocardiogram should be investigated to identify underlying Kounis syndrome. In addition to adrenaline, coronary dilators are the definitive treatment. Nicorandil may be a useful treatment option because of its minimal effect on blood pressure.

Rare and unexpected ventilation difficulties due to tracheal diverticulum: A case report.
Tracheal diverticulum is a rare airway-related particular occurrence, and the forcible tube insertion may cause tracheal ruptures during tracheotomy. Therefore, fiberoptic bronchoscopy (FOB) should be used routinely on all patients undergoing tracheal intubation or tracheotomy. ⋯ Tracheal diverticulum is an infrequent cause of tube inserting difficulty for the tracheotomy, and FOB is the first option for patients with catheter placement difficulty and mechanical ventilation difficulty.

Serum alkaline phosphatase (ALP) plays an important role in bone metabolism. However, the association between serum ALP and bone mineral density (BMD) remains inconclusive. Therefore, this study aimed to explore the relationship between serum ALP levels and pelvic BMD in young adults. ⋯ Additionally, smooth curve fitting and threshold effect analysis showed a persistent negative correlation between serum ALP and pelvic BMD, with a saturation effect at 97 U/L. Our results revealed a negative correlation between serum ALP levels and pelvic BMD in young adults. Monitoring serum ALP levels could help in the early detection of risks for bone metabolic disorders such as osteoporosis.

Efficacy of Baduanjin for treatment of fatigue: A systematic review and meta-analysis of randomized controlled trials.
Numerous studies have suggested that Baduanjin, a traditional Chinese exercise, can alleviate fatigue symptoms in patients with various illnesses. The aim of this review was to evaluate the efficacy of Baduanjin in reducing fatigue symptoms. ⋯ Baduanjin can effectively alleviate fatigue symptoms with relatively flexible requirements. However, studies investigating the same disease types and including non-Chinese populations are scarce. Therefore, further studies with long-term interventions, larger sample sizes, and well-designed methodologies are warranted.
