What Is Known About Preventing, Detecting, and Reversing Prescribing Cascades: A Scoping Review.
To systematically describe the resources available on preventing, detecting, and reversing prescribing cascades using a scoping review methodology. ⋯ Prescribing cascades are a recognized problem internationally. By learning from the range of resources to prevent, detect, and reverse prescribing cascades, this review contributes to improving drug prescribing, especially in older adults. To study the effect of caregiver-focused interventions to support medication safety in older adults with chronic disease. ⋯ Although some interventions improved caregiver medication knowledge and self-efficacy, effects on clinical outcomes and healthcare use were insufficiently studied. Two studies implementing collaborative care models with medication management components showed potential for improvement in quality of clinical care and reductions in healthcare visits and warrant further study with respect to medication safety. To characterize antibiotics prescribed to older adults to guide efforts to improve antibiotic use. ⋯ On average, in 2014, U.S. adults aged 65 and older received enough outpatient antibiotic courses for every older adult to receive at least 1. Quinolones and azithromycin are potential targets for assessing the appropriateness of antibiotic prescribing in this population. Interventions to improve use targeting internists and family physicians in the South Census region might have the potential to have the greatest effect.
