Case ReportsSurgical emphysema and pneumomediastinum complicating dental extraction.
Subcutaneous and mediastinal emphysema is a rare complication of dental extraction and the use of air turbines has often been implicated. We describe a case which highlights a serious complication of the use of an air rotor for the removal of a right second mandibular molar.

Appropriateness of a Resuscitation Council (UK) advanced life support course for primary care dentists.
To investigate whether a standard Resuscitation Council (UK) ALS course is appropriate for primary care dentists or whether a course should be specifically designed for dentists. ⋯ Despite rating some aspects as unimportant, all dentists stated that this course had been appropriate. They did not want a specially designed ALS course for dentistry. Taking exactly the same recognised course and assessments as other healthcare professionals and gaining the same certification was felt to be important to this group of dentists.

ReviewDental treatment for people with challenging behaviour: general anaesthesia or sedation?
The dental care of people with severe learning disability and challenging behaviour presents many problems. The maintenance of oral health by regular examination, prevention and treatment may be difficult because of the limitations in patient cooperation. In many cases the diagnosis of orofacial pain may need to be discounted as a cause of uncharacteristic and sometimes aggressive behaviour. ⋯ The use of novel techniques of sedation combining intravenous with oral or intranasal routes is described with patients treated in a community dental health centre. All patients had previously received treatment using general anaesthesia. The techniques described proved effective and safe for use in the primary care setting.

To investigate the effect of midazolam and flumazenil on psychomotor function and alertness in human volunteers. ⋯ An earlier discharge time based on subjective assessment of alertness is not advocated for patients whose intravenous midazolam sedation is reversed with flumazenil.
