First dorsal compartment anatomy was analyzed for the presence of a separate compartment for the extensor pollicis brevis (EPB) tendon and the ability of the EPB to extend the thumb interphalangeal (IP) joint in order to determine if these characteristics were associated with each other, and with de Quervain's disease. ⋯ In a substantial number of people undergoing surgery for de Quervain's disease and in cadavers, the EPB can extend the thumb IP joint. When it does, particularly in patients with de Quervain's disease, it is likely to reside in a subcompartment of the first dorsal compartment. The incidences of a subcompartment for the EPB and the ability of the EPB to extend the thumb IP joint were higher in the de Quervain's patient population than in the cadaver group.

A patient's reaction to a perceived increase in work demand may be related to his or her upper limb symptoms. The purpose of this study was to determine whether a brief measure of a patient's perception of how they respond to perceived increases in demands at work predicts pain levels and work status 6 months after an initial consultation with a hand surgeon. ⋯ Prognostic IV.

Vascularized bone grafting has been proposed as a treatment for scaphoid nonunions with avascular necrosis of the proximal pole. The purpose of this investigation is to report the results of vascularized bone graft and internal fixation for established scaphoid nonunions with proximal pole avascular necrosis as measured by validated outcome instruments. ⋯ The results of this investigation support the use of a vascularized bone graft for the treatment of scaphoid nonunions with avascular necrosis of the proximal pole.
