Longitudinal instability of the forearm resulting from an Essex-Lopresti injury is a surgical challenge, and no technique has yet met universal success. A new technique is presented here consisting of reconstruction of the radial head, leveling of the distal radioulnar joint, reconstruction of the central band of the interosseous membrane by using a pronator teres rerouting technique, and finally repair of the triangular fibrocartilage complex. It is hoped that by addressing all of the contributing longitudinal stabilizing structures, the longitudinal instability of the forearm will be controlled. The technique is challenging and requires much surgical experience.

Unstable fracture-dislocations of the proximal interphalangeal (PIP) joint remain a difficult management problem, often leading to residual pain, stiffness, and recurrent instability. In a military setting, an easily applied, simple to operate, and inexpensive device becomes an attractive option. The purpose of this clinical investigation was to retrospectively review use of dynamic distraction external fixation (DDEF) for unstable fracture-dislocations and pilon injuries of the PIP joint in an active-duty population. ⋯ Therapeutic IV.

The purpose of this study was to evaluate the treatment of patients with Mason type III radial head fracture with and without elbow dislocation by open reduction with internal fixation, collateral ligament repair, and early mobilization. An additional purpose was to investigate whether there is any effect of elbow dislocation on the severity and functional outcome. ⋯ Prognostic II.
