[Prognostic value of electrocorticography in temporal lobe epilepsy: patterns of relationing mesial and neocortical activity].
Electrocorticography (ECoG) monitoring in temporal lobe epilepsy (TLE) has been employed since the 40 s as a means to delineate surgical removal especially in lesional epilepsy, to reduce resection size and decrease cognitive sequelae (memory, naming). However in recent years, ECoG has been claimed to lack indications and prognostic value in cases of non lesional TLE. On the grounds of the pathophysiological relationship between mesial structures and neocortex (through propagation pathways) we have suggested a classification of ECoG activity patterns regarding the activities simultaneously recorded in mesial and neocortical grids. ⋯ ECoG reflects the state of pathological involvement of neocortex and mesial structures. An expertise approach, although brief in time (20-30 min), may either lead to preservation of relevant tissue (patterns I, II) and to establish a prognosis on the grounds of the initial epileptogenic activity.

Randomized Controlled Trial Clinical Trial[Use of procholinergics in the prevention of postoperative delirium in hip fracture surgery in the elderly. A randomized controlled trial].
40 to 50% of elderly with hip fracture present delirium. The morbimortality increase in patients whose presented delirium. ⋯ In the present study the citicoline did not prevent or reduce the incidence of delirium in hip fracture surgery in elderly.

[Monitorization of the jugular oxygen saturation and cerebral ischaemia in the neurocritical patient].
Catheterization of the jugular bulb is of great value in the study of cerebral metabolism, since it permits diagnosis of episodes of hypoxia and cerebral ischaemia. ⋯ Episodes of desaturation are frequent in neurocritical patients and are associated with increased mortality. Recordings of lactate are not useful at present in the diagnosis of cerebral ischaemia.

Review[Classification of the cerebrovascular diseases. Iberoamerican Cerebrovascular diseases Society].
In view of the several terminologies in countries belonging to the Iberoamerican Cerebrovascular diseases Society it seemed appropriate that the Executive Committee should propose a unified cerebrovascular diseases classification and nomenclature. ⋯ We consider that this nomenclature is simple and easy to use and in concordance with the international classifications, which permits the use of common terminology that is easily translated into English.

To define the concepts of orphan drugs and diseases, the current situation of clinical investigation of them, the attitude of the pharmaceutical industry and aspects of health legislation, especially in the European Community. ⋯ The struggle to treat rare diseases must continue, since its true importance is shown when suffered personally.
