Statins are among the most frequently prescribed drugs in Germany. Their benefits in lowering cardiovascular risk are beyond dispute. ⋯ Despite their relative frequency, it is difficult to objectively diagnose them, as the time until appearance of first symptoms, the nature of the complaints and the severity of muscle problems vary widely. This narrative review summarizes the causes of SAMS as well as new possibilities regarding their diagnosis and therapy.

A 39-year-old male patient presented with dyspnea and palpitations. The electrocardiogram (ECG) showed a first degree atrioventricular (AV) block and frequent atrial extra systoles. ⋯ The serological proof of antibodies confirmed the diagnosis of Lyme carditis and antibiotic treatment with ceftriaxon i.v. was initiated. Normal AV conduction finally resumed and the symptoms subsided completely without any further clinical consequences.

[Acute coronary lithotripsy in an 83-year-old man with syncope and acute coronary syndrome].
An 83-year-old male presented with syncope and acute coronary syndrome. An electrocardiogram (ECG) suggested left main (LM) coronary artery stenosis; angiography confirmed a heavily calcified tight LM stenosis. ⋯ Intravascular lithotripsy is a new and easily applicable method for heavily calcified coronary lesions. Cost-effectiveness and long-term results will define its future role.
