Reconstruction of the cranial base is often necessary after transsphenoidal surgery to prevent the occurrence of cerebrospinal fluid rhinorrhea and to maintain anatomic integrity. In most cases, sellar packing (fat, muscle, gelatin sponge) may be supported by bone or cartilage harvested at the time of surgery. The use of synthetic material, however, becomes desirable in cases in which an autograft is not available. Low-molecular-weight polylactide implants may serve as an effective alternative because they are immunologically inert, magnetic resonance imaging-compatible, and easily contoured to custom-fit a defect. ⋯ Polylactide polymer implants are effective adjuncts in transsphenoidal surgery when cranial base reconstruction is necessary and when an endogenous osseous or cartilaginous graft is unavailable.

Transcallosal resection of hypothalamic hamartomas, with control of seizures, in children with gelastic epilepsy.
Hypothalamic hamartomas (HHs) are associated with precocious puberty and gelastic epilepsy; the seizures are often refractory to antiepileptic medications and associated with delayed development and disturbed behavior. The current opinion is that surgery to treat intrahypothalamic lesions is formidable and that complete excision is not technically achievable. We report our experience with a transcallosal approach to the resection of HHs. ⋯ Complete or nearly complete resection of HHs can be safely achieved via a transcallosal approach, with the possibility of seizure freedom and neurobehavioral improvements.

Surgical relationship of the medial pectoral nerve to the musculocutaneous nerve: a cadaveric study.
For purposes of neurotization of the musculocutaneous nerve (MCN) with the medial pectoral nerve (MPN) after upper trunk brachial plexus injuries, the anatomic relationship between these two nerves was defined in a cadaveric model. ⋯ When planning to use the MPN for neurotization of the MCN, one should be prepared to harvest an interposition graft, because over one-third of MPNs may not have enough length to reach the MCN in a tension-free manner. Diameter mismatch occurs predictably between the distal MPN and the proximal MCN.

The presentation, screening, management, and clinical outcomes of patients who presented to our institution from 1973 to 1999 with central nervous system (CNS) hemangioblastomas in von Hippel-Lindau (VHL) syndrome and sporadic disease were analyzed. ⋯ Surgical outcomes for patients with CNS hemangioblastomas are favorable. However, management of hemangioblastomas is a more difficult and prolonged endeavor for patients with VHL syndrome. In patients with VHL syndrome, neuroradiological screening allows identification of lesions before they become symptomatic. Because patients with VHL syndrome are at risk for development of new lesions, they require lifelong follow-up.
