Comparative StudyAAIsafeR limits ventricular pacing in unselected patients.
Dedicated pacing modes, such as AAIsafeR, prevent ventricular (V) pacing in selected patients. We report our experience in consecutive unselected patients. ⋯ In unselected pacemaker recipients, AAIsafeR reliably prevented V pacing compared with other pacing modes. No adverse effects were reported by any patient. Furthermore, maintaining spontaneous AV conduction protected the patients against ELT episodes.

Clinical TrialFeasibility of temporary biventricular pacing in patients with reduced left ventricular function after coronary artery bypass grafting.
Biventricular pacing improves hemodynamics after weaning from cardiopulmonary bypass in patients with severely reduced left ventricular (LV) function undergoing coronary artery bypass grafting (CABG). We examined the feasibility of temporary biventricular pacing for 96 hours postoperatively. Unipolar epicardial wires were placed on the roof of the right atrium (RA), the right ventricular (RV) outflow tract, and the LV free lateral wall and connected to an external pacing device in 51 patients (mean LV ejection fraction 35 +/- 4%). Pacing and sensing thresholds, lead survival and incidence of pacemaker dysfunction were determined. ⋯ Biventricular pacing after CABG using a standard external pacing system was feasible and safe.

Clinical TrialG-CSF-induced mobilization of CD34(+) progenitor cells and proarrhythmic effects in patients with severe coronary artery disease.
Granulocyte colony stimulating factor (G-CSF) therapy has been reported to be proarrhythmic. The in vivo mobilization of endothelial progenitor cells (EPCs) and the possible proarrhythmic effects in patients with severe coronary artery disease (CAD) and inducible ischemia have not been described. ⋯ This is the first study showing that mobilization of CD34(+) EPCs is safe in patients with severe CAD. The accompanying leukocytosis did not appear proarrhythmic. Changes in P wave duration might be attributable to G-CSF therapy.

Cardiac resynchronization therapy optimization by ultrasonic cardiac output monitoring (USCOM) device.
We investigated the accuracy and feasibility of a 2D echo-independent ultrasonic continuous wave Doppler cardiac output monitoring device (USCOM) operated by trained nurse for the atrio-ventricular interval (AVI) optimization in cardiac resynchronization therapy (CRT). ⋯ USCOM device operated by trained nurse can provide a simple, accurate, and fast non-invasive method for the AVI optimization in CRT population.
