The authors hypothesized that sepsis workup recommendations are associated with practice recommendations published during the physician's residency. The first published recommendations suggesting sepsis workups for nontoxic, young, febrile infants appeared in pediatric journals from 1975-1980 and in family practice journals from 1981-1987. Data are from the Community Tracking Study (3,272 pediatricians and 2,432 family physicians). "Percentage of sepsis workups recommended" was defined by response to a vignette about the percentage of well-appearing 6-week-old children with a fever of 101 degrees F for whom the physician would recommend a sepsis workup. ⋯ Pediatricians recommended sepsis workups 81.6% of the time and family physicians 67.7% (p < .001). Increased recommendations occurred among pediatricians who completed residency from 1975-1980 (p < .05) and among family physicians who completed residency from 1981-1987 (p < .005), when specialty-specific journals published recommendations for sepsis workups for febrile infants. The association between publication of sepsis workup recommendations during a physician's residency and increased sepsis workup recommendations suggests an unrecognized and enduring impact of such publications.

Standardized patient examinations are being used for high-stakes decisions (e.g., graduation, licensure, and certification) with growing frequency. Concurrently, research on methods to determine the passing score for these types of performance-based assessments has increased. A wide variety of approaches have been considered in the past several years, many based on traditional techniques developed for use with multiple-choice examinations. ⋯ The purpose of this article is to describe and evaluate a work-centered approach for determining the passing score for a performance-based assessment of clinical skills. A description of an approach to set passing scores that involves expert review of examinee work is presented. Application and evaluation of the method are illustrated using examination materials obtained from a high-stakes certification assessment.

Is acupuncture analgesia an expectancy effect? Preliminary evidence based on participants' perceived assignments in two placebo-controlled trials.
This purpose of this article is to contrast the analgesic efficacy of acupuncture following dental surgery with the analgesic effects based on the expectation of benefit in two independently conducted placebo-controlled trials evaluating acupuncture as an adjunctive therapy for dental surgery. Both trials used pain following dental surgery as the outcome variable, and both included a blinding check to ascertain patients' beliefs regarding which treatment they were receiving. ⋯ Patients' beliefs regarding the receipt of acupuncture bore a stronger relationship to pain than any specific action possessed by acupuncture. These results also support the importance of both employing credible controls for the placebo effect in clinical trials and evaluating the credibility of those controls.

This article describes the adaptation of the Finnish 15D standardized measure of health-related quality of life (HRQoL) instrument for use in Turkey and assesses its psychometric properties. The HRQoL is measured in a sample of 75 patients with Type 2 diabetes using both 15D and Nottingham Health Profile (NHP) instruments. The internal consistency within the domains of 15D was high, with Cronbach's alpha values 0.89 for 15D scale and 0.89 for NHP. ⋯ Overall, the results indicated that the adaptation of the 15D for use in Turkey was successful. The Turkish version was found to be a reliable and valid instrument. It is suitable and applicable to both clinical and population-based studies for the measurement of HRQoL in Turkey.

Despite the wide availability of statistical programs designed to deal with longitudinal data from a multilevel perspective, many applied researchers remain unfamiliar with the benefits of this methodology, particularly for the evaluation of interventions. The authors present an example of multilevel modeling as part of the analysis of evaluation data from an HIV intervention study. ⋯ Multilevel analysis results are compared and contrasted with typical approaches through general linear models for repeated-measures data. Analyses are presented using the SPSS and HLM 5 software.
