The study aimed to investigate the association between lipocalin-2 (LCN-2) levels and diabetic retinopathy (DR) in patients with overweight/obese type 2 diabetes mellitus(T2DM), and to explore the mechanism of LCN-2 in overweight/obese DR. ⋯ LCN-2 levels in overweight/obese groups were higher than those in normal groups (P < 0.001 for all), and patients with DR had higher levels of LCN-2 than those without DR(P < 0.05 for all) in normal groups and overweight/obese groups. Binary logistic regression analysis showed that no significant significance was observed for LCN-2 levels compared to those below the median in the normal group, but individuals with LCN-2 levels above the median had 4.198 times higher risk of developing DR than those below the median (OR = 4.198, 95% CI = 1.676-10.516) after adjustment for potential confounding factors in the overweight/obese group. In the total, normal and overweight/obese groups, the prediction capacity of LCN-2 for DR was 1.56, 1.58 and 1.65 times, respectively. Conclusionsː In conclusion, our study found that LCN-2 levels were higher in overweight/obese patients with DR, and LCN-2 was an independent predictor of DR in T2DM patients with overweight/obese. In addition, LCN-2 may be a valuable predictor of DR-like factors such as the duration of diabetes and hypertension.

Idiopathic granulomatous mastitis (IGM) is a rare inflammatory breast disease, in which there is no clear established treatment algorithm. Several physicians keep away from using immunosuppressive (IS) treatments in routine clinical practice. ⋯ IS agents provide high rate of prolonged drug-free remission and should be considered a part of routine medical care of the patients with IGM.

Atrial fibrillation is the most common cardiac arrhythmia and is a major risk factor for stroke, with an incidence rate of 4-5% per year. The use of DOACs is recommended for specific patient populations however the risk of bleeding commonly precludes their use. Left atrial appendage occlusion is a relatively novel procedure recommended for such patients. We set out to analyse the initial success and safety of this procedure in a single site. ⋯ We report lower technical success and procedural success rates in an older population cohort than historically studied, 90% of whom had an absolute contraindication to oral anticoagulation, with higher CHADS2VaSc and HASBLED scores than commonly studied.

Uremic pruritus: prevalence, determinants, and its impact on health-related quality of life and sleep in Indian patients undergoing hemodialysis.
Uremic pruritus has an impact on the quality of life and sleep of hemodialysis patients, but the majority of cases go unreported and untreated unless severe, due to a lack of awareness. The purpose of this study is to determine the prevalence, associated factors, and impact on health-related quality of life (HR-QOL) and sleep in hemodialysis patients. ⋯ The majority of patients on maintenance hemodialysis experience uremic pruritus. None of the clinical characteristics, dialysis-related factors, and laboratory parameters affected uremic pruritus. Uremic pruritus patients had the worst HR-QOL & their sleep quality significantly declined as pruritus severity escalated.

There are limited studies examining alcohol consumption in Gaelic Athletic Association (GAA) players. In a previous paper, we reported excess alcohol consumption, alcohol-related harms and binge drinking amongst elite GAA players. In that survey, the players were provided with an opportunity to provide comments on alcohol. This current study analyses these comments. ⋯ These data show players recognise intermittent binge drinking with periods of abstinence and alcohol-related harms. Further initiatives regarding alcohol harm reduction merit consideration including prohibition of alcohol sponsorship, similar to the GAA's ban on gambling.
