Comparative StudyRisk factors for acute kidney injury following TA-TAVI or minimally invasive aortic valve replacement: which procedure is less kidney damaging in elderly patients?
Acute kidney injury (AKI) represents a major complication following aortic valve replacement in elderly patients. The aim of this study was to determine possible risk factors for AKI and to find the ideal strategy, minimally invasive valve replacement (MIS-AVR) or transapical valve implantation (TA-TAVI), regarding the postoperative renal outcome. ⋯ A higher risk for AKI after TA-TAVI should be considered in the therapy decision, especially in elderly male patients because MIS-AVR still yields excellent results.

Natural course of spontaneous pneumothorax without bullae or blebs under high-resolution computed tomography.
The cause of primary spontaneous pneumothorax (PSP) is known as rupture of some bullae or blebs. ⋯ Several bullae and blebs were revealed upon operation of PSP without previously detected bullae and blebs on HRCT. We cautiously recommend operating on PSP regardless of whether bullae and blebs are detected by HRCT as long as there are no contraindications to the operation.

Case ReportsTension subcutaneous emphysema during laparoscopic surgery treatment of colon cancer: a case report.
Carbon dioxide (CO2) insufflation is now essential for most endoscopic surgeries, such as abdominal, pelvic, and neck endoscopic surgery. It is not uncommon for CO2 leaks to occur unintentionally into subcutaneous tissue, later diffusing into a patient's bloodstream and resulting in hypercarbia. ⋯ Herein, we report on a case of tension subcutaneous emphysema and subsequent fatal ventilatory failure due to massive subcutaneous emphysema during laparoscopy. A timely blowhole incision is an effective intervention in an emergent setting like this case, although the patient had endotracheal intubation.
