Randomized Controlled Trial Comparative Study Clinical TrialA comparison of 2 analgesic regimens for the control of postoperative periodontal discomfort.
A single blind study of 24 patients compared the postoperative periodontal pain relief and adverse effects associated with a pretreatment regimen with etodolac, a nonsteroidal anti-inflammatory drug (NSAID), to a typical pro re nada (prn) regimen with a combination of acetaminophen with hydrocodone. Patients selected required one or more periodontal osseous surgeries that were judged to involve relatively similar degrees of surgical manipulation. Patients in the etodolac group received two 300 mg capsules 30 minutes prior to surgery and then redosed themselves prn. ⋯ The side effects were minimal for both of the drugs studied. It was concluded that the analgesic regimens tested under clinical practice conditions were comparable in providing analgesia with minimum side effects in uncomplicated periodontal osseous surgery. Studies with larger numbers of patients are needed to definitively address whether these regimens are truly equivalent.

Periodontal effects associated with the use of smokeless tobacco.
This report describes periodontal findings from a comprehensive study of smokeless tobacco use in professional baseball players. Subjects consisted of 1,094 players, coaches, and training staff of seven major league and their associated minor league teams. Before being examined, subjects completed questionnaires on patterns of smokeless tobacco use (validated by blood chemistry studies), rinsed their mouths under supervision, and were cautioned not to discuss their use of tobacco with the dental examiners. ⋯ Among current week users, 46% had oral mucosal lesions, located primarily in the mandible at sites where the smokeless tobacco quid was placed. The use of smokeless tobacco was not necessarily associated with severe forms of periodontal disease, and the presence of poor oral hygiene and gingivitis in these users was not related to the development of oral lesions. However, sites adjacent to mucosal lesions in smokeless tobacco users showed significantly greater recession and attachment loss than in sites not adjacent to lesions in users or comparable sites in non-users.

Comparative Study Clinical Trial Controlled Clinical TrialAnalgesic efficacy of flurbiprofen as compared to acetaminophen and placebo after periodontal surgery.
The purpose of the present investigation was to compare the analgesic efficacy of flurbiprofen, a nonsteroidal anti-inflammatory drug, which is a phenyl propionic acid derivative, with acetaminophen and placebo in 63 adult outpatients with moderate to severe dental pain following periodontal surgery. After surgery was completed under local anesthesia, the patients received under double-blind conditions an envelope containing four tablets of either flurbiprofen 100 mg, acetaminophen 500 mg, or placebo and they were instructed to take one tablet every 6 hours when postoperative pain reached moderate to severe intensity. ⋯ Flurbiprofen was shown to possess an adequate analgesic effect superior to either placebo (P less than 0.005) or acetaminophen (P less than 0.01) in the parameters studied. Our results seem to further support earlier data obtained with the drug in dental patients with postoperative pain after the surgical removal of impacted third molars; therefore, it is concluded that flurbiprofen used as directed is a new alternative for the proper treatment of pain following periodontal surgery.

In vitro effectiveness of a newly-designed ultrasonic scaler tip for furcation areas.
This study was undertaken to perform an in vitro comparison of the accessibility and effectiveness of a newly-designed furcation tip for an ultrasonic system with conventional curets and ultrasonic scalers. The new furcation tip was spiral in shape with a spherical end. The study was conducted on three groups of 40 teeth, each comprising 20 maxillary and 20 mandibular molars. ⋯ The newly-designed tip thus showed high levels of effectiveness and accessibility for furcation debridement. Although the SEM photographs showed that each instrument had affected the furcation surfaces in various degrees, the teeth instrumented by the newly-designed tip maintained relatively smoother surfaces. It was concluded that the newly-designed furcation tip was more effective than the straight type ultrasonic scaler or the Gracey curet in the debridement of furcation in vitro.

Case ReportsPeriodontal manifestations of hyperoxaluria and oxalosis.
Dental and periodontal findings associated with primary hyperoxaluria in a 29-year old male patient are described. This is a rare, inherited, metabolic disease which results in excessive calcium oxalate synthesis. The predominant and early manifestation of hyperoxaluria is nephrocalcinosis which results in chronic renal failure. ⋯ Extensive infiltration of crystals was noted in the pulps of the teeth, in the marrow spaces of the alveolar bone, in the gingival corium, and in the periodontal ligament. Crystalline calcium oxalate deposits in the periodontal ligament provoked a granulomatous foreign-body reaction. This resulted in aggressive external root resorption leading to pulp exposure and tooth mobility.
