Review Case ReportsLipoatrophia semicircularis: a case report and review of the literature.
The case of a 45 years old woman with lipoatrophia semicircularis is presented showing a typical semicircular depressions of the anterolateral aspects of the thighs. The lesion regressed after avoiding the pressing of the thighs against the desk edge. A PubMed search disclosed 125 cases of lipoatrophia semicircularis reported in details. ⋯ Repeated external microtraumatisms was the most common cause. The few cases biopsied revealed a localized lipoatrophy. Avoiding microtraumatisms resulted in a complete remission of lipoatrophia semicircularis in most cases.

Case ReportsUse of infliximab in toxic epidermal necrolysis: a still opened challenge.
Toxic epidermal necrolysis (TEN), also known as Lyell syndrome, is a potential life-threatening muco-cutaneous disease with important systemic implications. It affects the skin and mucous membranes, with involvement of more than 30% of body surface and it is mostly caused by drugs. ⋯ Currently, there are no randomized control trials and stardardized therapeutical approaches for the management of Lyell syndrome; therefore controversial clinical responses to the most common used drug in TEN make it difficult for the clinical-therapeutic approach. The authors reported their experience on three patients affected by Lyell syndrome treated with infliximab.

Inherited epidermolysis bullosa (EB) refers to a clinically and genetically heterogeneous group of rare disorders characterized by fragility of the skin and mucous membranes. Despite the preclinical development of different molecular and cell-based treatment strategies, no cure is still available for EB. Therefore it is important to establish the management of EB patient since neonatal age in order to ensure an early diagnosis and contribute to prevent complications and to improve quality of life. ⋯ A well-organized and structured continuity of care is important in EB like in all chronic and rare diseases. The management of EB patients is very important because a specific attention to the fragile skin is required in order to reduce pain, risk of trauma, ulceration and infection. The center of expertise stays as the main structure offering specialized care to EB patients and also helps patients and their families to understand and deal with the disease and to ensure an adequate liaison with the community healthcare system.

Clinical TrialThe use of skin needling for the delivery of a eutectic mixture of local anesthetics.
The use of skin needling is believed to aid the transdermal delivery of drugs, even if it is mostly used for skin collagen induction. The aim of this paper was to use skin needling, combined with a local anesthetic EMLA (eutectic mixture of lidocaine and prilocaine), as a way to enhance transdermal drug penetration and optimize the analgesic effects of common local anesthesia. ⋯ The use of skin needling can improve the transdermal delivery of an emulsion-like eutectic mixture of local anesthetics (EMLA) and can introduce the use of this method for delivering topical molecules in dermatology.

Case ReportsAcute generalized exanthematous pustulosis: report of five cases and systematic review of clinical and histopathological findings.
Acute generalized exanthematous pustulosis (AGEP) is a rare, drug-related pustular eruption usually starting from folds with edema and erythema and with subsequent spreading. Clinically AGEP is characterized by the sudden appearance of dozen of sterile, non follicular, small pustules on erythematous and edematous skin. Mild non erosive mucosal involvement, mostly oral, may sometimes occur. ⋯ Although AGEP is a self-limiting disease with a favourable prognosis, secondary infections are a not infrequent complication in patients in poor general medical conditions. The reported mortality is about 5%. The most severe cases are associated with drug rechallenge.
