Neutrophil-gelatinase-associated lipocalin and renal function after percutaneous coronary interventions.
The value of neutrophil-gelatinase-associated lipocalin (NGAL), a novel biomarker in the detection of acute renal failure in children after cardiac surgery, has been highlighted in previous studies. The incidence of percutaneous coronary intervention (PCI) increases, which may possibly result in increased incidences of contrast nephropathy, its potentially serious complication. Therefore, the aim of our study was to assess prospectively NGAL in patients undergoing elective PCI in relation to serum creatinine. ⋯ NGAL may represent a sensitive early biomarker of renal impairment after PCI. Serum creatinine, duration of PCI, but not type and amount of contrast agent, and appropriate treatment of diabetes, reflected by HbA1c, predict a rise in serum NGAL and kidney function following PCI.

The lack of early biomarkers for acute renal failure (ARF) has crippled our ability to launch potentially effective therapeutic measures. We tested the hypothesis that urinary proteomics could identify novel early biomarker patterns for ischemic renal injury. ⋯ SELDI-TOF-MS is a novel, non-invasive, sensitive, highly predictive, reproducible, rapid method for the prediction of acute renal injury following CPB.

Cisplatin is one of the most widely used chemotherapeutic agents, but the risk of nephrotoxicity frequently hinders the use of higher doses to maximize its antineoplastic effects. The lack of early biomarkers has impaired our ability to initiate potential therapeutic or preventive interventions in cisplatin nephrotoxicity in a timely manner. In this study, we have explored the expression and urinary excretion of neutrophil gelatinase-associated lipocalin (NGAL) in a mouse model of cisplatin-induced nephrotoxic injury. ⋯ The results indicate that NGAL represents an early and quantitative urinary biomarker for cisplatin nephrotoxicity.

Apoptosis plays an important role in the morphogenesis of the renal papilla. During kidney development, ATL is derived from the TAL in the inner medulla by apoptotic deletion of a fraction of TAL cells and the transformation of the remaining TAL cells. EGF is an important regulator of apoptosis in the kidney. ⋯ These findings suggest that exogenous EGF delays the development of loop of Henle in the renal papilla by reducing both apoptosis and cell proliferation.

Epidemiologic data regarding the prevalence of chronic renal insufficiency (CRI) [from the third National Health and Nutrition Examination Survey (NHANES III)] and the incidence of end-stage renal disease (ESRD) [from the United States Renal Data System (USRDS)] are available. However, reconciliation of these separate particulars has not been performed objectively. The present work examines the epidemiology of CRI of nondiabetic etiology and ESRD in black and white Americans aged 20 years or greater. ⋯ There is a major discrepancy in the epidemiology of nondiabetic CRI and ESRD amongst black and white Americans. The reasons for this need further study.
