Hydrotherapy, balneotherapy, and spa treatment in pain management.
The use of water for medical treatment is probably as old as mankind. Until the middle of the last century, spa treatment, including hydrotherapy and balneotherapy, remained popular but went into decline especially in the Anglo-Saxon world with the development of effective analgesics. ⋯ It is our opinion that the three therapeutic modalities must be considered separately, and this was done in the present paper. In addition, we review the research on the mechanism of action and cost effectiveness of such treatments and examine what research might be useful in the future.

Investigation of the effects of pool-based exercise on fibromyalgia syndrome.
The aim of this study was to compare pool-based exercise and balneotherapy in fibromyalgia syndrome (FMS) patients. ⋯ The results of our study showed that pool-based exercise had a longer-lasting effect on some of the FMS symptoms, but statistical analysis failed to show a significant superiority of pool-based exercise over balneotherapy without exercise. While we believe that exercise is a gold standard in FMS treatment, we also suggest in light of our results that balneotherapy is among the valid treatment options in FMS, and further research regarding the type and duration of the exercise programs is necessary.

A pilot trial of quantitative Tc-99m HMPAO and Ga-67 citrate lung scans to detect pulmonary vascular endothelial damage and lung inflammation in patients of collagen vascular diseases with active diffuse infiltrative lung disease.
Diffuse infiltrative lung disease (ILD) includes a heterogeneous group of disorders predominantly affecting lung parenchyma and sparing the airway. To assess the degree of pulmonary vascular endothelial damage in active ILD, lung/liver uptake ratios (L/L ratio) on technetium-99m hexamethylpropylene amine oxime (Tc-99m HMPAO) lung scan were determined in 30 patients with active ILD. Meanwhile, the gallium-67 citrate (Ga-67) uptake index (GUI) on Ga-67 lung scan was used to evaluate the severity of lung inflammation in active ILD. ⋯ In addition, there were no good correlations between HRCT scoring vs GUI and HRCT vs L/L ratio in different study subgroups. In conclusion, L/L ratios on Tc-99m HMPAO and GUI on Ga-67 lung scans differ from chest X-ray findings and have the potential to detect the degree of pulmonary vascular endothelial damage and severity of lung inflammation in active IDL. However, the relationships between HRCT scoring, GUI, and L/L ratio in patients with collagen vascular diseases and active ILD are not significant.

Irritable bowel syndrome (IBS) and fibromyalgia (FM) are common functional diseases in adult women. The aim of this study was to investigate whether female predominance in IBS is related to FM. Fifty patients with IBS and 50 healthy controls were enrolled. ⋯ Comparison of IBS+FM and IBS-only patients showed no significant difference in depression and anxiety status. However, both anxiety and depression scores were found to be higher in female IBS patients than their male counterparts ( P<0.01 and P<0.05, respectively). Our findings suggest that the female predominance in IBS patients may result from coexisting FM.

In this study, serum antioxidant and oxygen derived free radical status of patients with ankylosing spondylitis (AS) was investigated and compared with that of age- and sex-matched healthy controls. The relationship of these parameters to disease activity indices was also examined. Thirty patients with AS not currently under disease-modifying antirheumatic drug (DMARD) treatment (e.g., sulfasalazine or methotrexate) (15 active and 15 inactive) and 16 age- and sex-matched healthy controls were included in the study. ⋯ Our results indicate that oxidative stress and lipid peroxidation are accelerated in untreated patients with active AS. Serum catalase activity may be closely related to disease activity. In this regard, we underscore the likely benefit of some therapeutic interventions including high-potential antioxidants that will potentiate the antioxidant defense mechanism and reduce peroxidation in the management of AS.
