We assessed the incidence, predictors, and outcomes of gastrointestinal bleeding (GIB) in patients with acute coronary syndromes (ACS). ⋯ GIB is a serious condition in the scenario of ACS and is independently associated with mortality and ischemic complications.

Multicenter StudyQuality of care for atrial fibrillation among patients hospitalized for heart failure.
This study sought to examine quality of care and warfarin use at discharge in patients with atrial fibrillation (AF) and heart failure (HF). ⋯ Guideline-recommended warfarin use in patients with AF and HF is less than optimal, has not improved over time, and varies significantly according to age, race, risk profile, region, and hospital site.

Randomized Controlled TrialNormal plasma levels of cardiac troponin I measured by the high-sensitivity cardiac troponin I access prototype assay and the impact on the diagnosis of myocardial ischemia.
This study sought to evaluate the analytical and clinical performance of the novel hypersensitive cardiac troponin I (cTnI) prototype assay from Beckman Coulter (Fullerton, California). ⋯ The novel high-sensitivity cTnI prototype assay from Beckman Coulter allows for the first time the measurement of cTnI levels in almost all healthy subjects. Our data indicate that the assay may be a powerful aid in the diagnosis and outcome prediction of patients with suspected myocardial ischemia and question any definition of myocardial infarction.

Usefulness of the valvuloarterial impedance to predict adverse outcome in asymptomatic aortic stenosis.
This study was designed to examine the prognostic value of valvuloarterial impedance (Z(va)) in patients with aortic stenosis (AS). ⋯ Increased Z(va) is a marker of excessive LV hemodynamic load, and a value >3.5 successfully identifies patients with a poor outcome. These findings suggest that beyond standard indexes of stenosis severity, the consideration of Z(va) may be useful to improve risk stratification and clinical decision making in patients with AS.
