Case Reports[Acute carpal tunnel syndrome and tendon rupture of the long flexor muscle of the thumb as a rare complication of a scaphoid pseudoarthrosis. Case report].
The authors demonstrate a case of a scaphoid pseudarthrosis leading to an attrition rupture of the flexor pollicis longus tendon, tenosynovitis and haemorrhage into the carpal tunnel causing recurrent symptoms of an acute carpal tunnel syndrome. Every acute carpal tunnel syndrome is an emergency requiring instant decompression of the carpal tunnel. If success with conservative treatment of acute carpal tunnel syndrome is questionable, early operative relief of the median nerve is the safest procedure.

[Radial island flap for the reconstruction of the 1st commissure].
The fasciocutaneous radial forearm island flap was used in six cases for reconstruction of the first web space after tumor resection, severe burns, a complex hand injury and a congenital adduction contracture of the thumb. Reconstruction of a deep and wide first web was possible in all cases. ⋯ Corrective surgery for an unsightly scarred donor area was considered in one case. The radial forearm island flap is therefore a reliable and low-risk procedure for coverage of first web defects.

Case Reports[Autologous injection of fatty tissue following liposuction--not a method for breast augmentation].
In recent years autologous fat transplantation employing liposuction has become an established method for correction of small soft tissue defects and irregularities of skin contour. Only a small amount of fat may be used because fat cells will be nourished by perifusion alone until neovascularisation occurs. Transplantation of a large amount of fat will result in fat necrosis. The inappropriate use of fat for breast augmentation after liposuction is described in a case report.

[Continuous axillary catheter plexus anesthesia--a method of postoperative analgesia and sympathetic nerve block following hand surgery].
During a period of one year (March 1985 to March 1986) 52 axillary brachial plexus catheters were left in place for two to twelve days. The main indications were postoperative analgesia and sympatholysis after microvascular surgery of the hand. With the continuous infusion of 0.25% or 0.375% bupivacaine, 6-8 ml/h a sufficient effect was seen. ⋯ There were no toxic side effects by the local anaesthetic agent. In one case an infection of the axillary region developed, which disappeared after removal of the catheter without any consequences. In summary the continuous axillary brachial plexus block is an acceptable method for intra and postoperative analgesia and sympatholysis in microvascular surgery of the hand.

Case Reports[Lengthening middle hand and finger segment stumps by external distraction devices following traumatic amputations and in congenital abnormalities].
Since November 1984, lengthening of metacarpals and digital phalanges by a distraction apparatus has been performed in six metacarpal bones and four phalangeal bones in nine patients (four females, five males). The age ranged from 14 to 59 years (average 36). The follow-up period was from two to 28 months (average 16). ⋯ Because of the relative simplicity of the method, no microsurgical technique is required. The main advantage of the procedure is that the lengthened digit retains its original sensation, vascular supply and some mobility. It is a useful procedure especially when a patient does not want to sacrifice another digit.
