Ultrasound-guided interventions are presently performed as free-hand-type procedures or using biopsy transducers. In this article we report on our experience with a new navigation-system for sonographically guided interventional procedures under OR-conditions. ⋯ The US-Guide 2000 trade mark as a virtual needle-guiding system allows a safe and accurate sonographically assisted intervention. The major advantage is the possibility of out-of-plane needle placement and the combination of flexibility of free-hand-type procedures with the accuracy of a biopsy transducer. This increases the safety of punctures especially when lesions are difficult to reach and/or are situated next to vulnerable structures. It also reduces the interventional trauma.

Transcutaneous ultrasound of pleural lesions has been established as a method complementary to conventional X-ray examination. The opacified lung allows the sonographical investigation of the lung parenchyma for synchronous and metachronous focal lesions. The aim of the pictorial essay is to describe the sono graphic patterns and to discuss the differential diagnosis, as well as the pathological correlation of focal lesions in the opacified lung.

Transcranial Doppler sonography was used to asses time relation and characteristically changes in middle cerebral artery blood flow velocity before syncope during tilt table test in children. ⋯ Patients with neurocardiogenic syncope do have characteristically blood flow velocity changes of middle cerebral artery just before loosing consciousness. Transcranial Doppler is a usefully examination during tilt table. Clear transcranial Doppler results can help to prevent children from the impending syncope.

Transglobe B-mode sonography of the optic nerve and its sheets is evaluated in respect to its value in detecting increased intracranial pressure. ⋯ A standardized transglobe sonogram of the optic nerve in combination with transcranial and abdominal sonography is a reliable method for the detection of elevated intracranial pressure.

Since 1995 sonography represents the diagnostic imaging procedure most commonly applied (52 %) by physicians working in the public health sector. On the account of the high cost of sonography (around 2,5 billion DM in 1998) it seems justified to intensify the quality control of advanced sonography training courses for physicians. This study was set out to evaluate the learning efficiency and teaching quality of sonography training courses for doctors. We also aimed at testing, evaluating and carefully optimising modern concepts for sonography training based on recognised didactic criteria. ⋯ Instructors of sonography courses need to be well trained in modern teaching methods. This study shows a variety of current concepts and provides a list of easily applicable didactic quality criteria for the conception of ultrasound courses.
