Bronchial artery embolization (BAE) is a well-established, non-surgical procedure in the emergency treatment of massive hemoptysis. This study aims to evaluate the immediate and long-term prognosis of BAE for the management of massive hemoptysis in our center. ⋯ BAE is an effective and safe intervention in cases of massive hemoptysis. However, recurrences are common and long-term follow-up is considered important with a view to perform repeated interventions with combination of embolic materials.

Changing trends in anti-coagulant therapies. Are heparins and oral anti-coagulants challenged?
The conventional management of thrombotic and cardiovascular disorders is based on the use of heparin, oral anticoagulants and aspirin. Despite progress in the sciences, these drugs still remain a challenge and mystery. The development of low molecular weight heparins (LMWHS) and the synthesis of heparinomimetics represent a refined use of heparin. ⋯ While clinically effective, recent results have prompted a closure of a clinical trial with Prasugrel due to bleeding. The newer anticoagulant and antiplatelet drugs are attractive, however, none of these are expected to replace the conventional drugs in polytherapeutic approaches. Heparins, warfarin and aspirin will continue to play a major role in the management of thrombotic and cardiovascular disorders for years to come.

Continuing medical education (CME) can be defined as ''educational activities that serve to maintain, develop, or increase the knowledge, skills and professional performance of a physician to provide services for patients, the public, or the profession''. CME is a major professional responsibility. The European Board of Vascular Surgery of the Union Européenne des Médecins Spécialistes (UEMS) Section of Vascular Surgery has, through its European Vascular CME (EVCME) Committee, accredited 74 congresses during the 5-year period from 2000-2004. ⋯ This survey indicates that the EVCME approved congresses had a positive impact for the vascular surgeon by updating overall knowledge on vascular surgery; the majority of comments by the participants also indicates that EVCME is fulfilling its aim to bring as much evidence-based practice as possible into the daily work schedule of the surgeon by turning knowledge acquired by CME into performance of the participants.

Intraoperative hypoperfusion ischemia is one factor that leads to perioperative stroke during carotid endarterectomy. Selective shunting is one way to reduce intraoperative hypoperfusion, but to shunt selectively needs a sensitive and simple monitoring system together with the rules for insertion. Near infrared spectroscopy (NIRS) is a monitoring system that can be used throughout the operation, and reflects the cerebral oxygenation instantly, but its value in insertion of shunt tubes is still controversial. The aim of this study was to determine criteria that can be used to insert shunt tubes. ⋯ NIRS monitoring is suitable for monitoring during carotid endarterectomy for selective shunting, because it can be used throughout the operation and shows the immediate change in oxygenation. There is a possibility that a decrease of 5% can be used as a decisive value for shunt insertion.
