[Indications of efficacy of bariatric surgery in the management of morbid obesity].
Its indications should only be considered in patients with morbid obesity (BMI>40 kg/m2) or severe obesity (BMI>35 kg/m2) with serious associated comorbidity. In general, significant weight loss with marked improvement of the cardiovascular comorbidity is obtained. There are still no studies that show long term survival. It must be stressed that the patients should follow a subsequent and continuous medical monitoring to prevent important secondary metabolic complications.

Phystestrogens are organic compounds produced by a large variety of plants with protective function against the invasion of them by microorganisms. Phystestrogens, and among them isoflavones, have a great similarity with estradiol, principal endogenous estrogen. ⋯ Due to the existence of contradictory data on the effects of soy isoflavones on the most outstanding health disorders of menopause such as osteoporosis and hot flushes, and its ineffective role in the improvement of the lipid profile, it is recommendable to continue choosing hygienic-dietary and conventional drug treatments. However, we could introduce soy and soy derived products within a balanced, varied and adequate diet for these women.

[Differential characteristics of primary acute pericarditis and severe pericardial effusion without tamponade].
The aim or our study was to evaluate the main clinical and etiological features or primary acute pericarditis with severe pericardial effusion without cardiac tamponade. Our experience included 35 patients with this problem diagnosed in the last 5 years. ⋯ Patients with severe pericardial effusion without cardiac tamponade who had a favourable clinical outcome on nonsteroidal antiinflammatory drugs therapy presented a lower proportion of specific etiology than those with a poor response to this therapy (8% versus 45%; p = 0.03). An exhaustive etiological investigation, including diagnostic pericardiocentesis should be reserved for these patients with unfavourable response to antiinflammatory drug therapy.

Review[ARB II in chronic heart failure. Coincidences and divergences. Class effect?].
Although it is possible to suggest that the ACE inhibitor may have a certain class effect in regards to the benefits observed in the treatment of chronic heart failure, it does not seem reasonable that the same can be assumed in the case of ARB II. Morbidity-mortality studies in chronic heart failure have only been conducted with three of the seven ARB II presently commercialized in our country. ⋯ In these clinical comments, we review the evidence available, stressing what effects could be common to all the ARB II, such as the prevention of diabetes or atrial fibrillation and what effects may only be attributable to some of these, at least up to now. However, new information of the already finished studies and some still on-going clinical trials may help us to know the effects of the ARB II in this disease more and better.
