Multicenter Study Comparative Study Clinical Trial[Diagnosis and ambulatory treatment of patients with community-acquired pneumonia: variability of complementary tests and study of the effectiveness of roxithromycin. Study Group on Ambulatory Treatment of Community-Acquired Pneumonia (NACTA)].
Community-acquired pneumonia (CAP) has been the subject of a number of studies. Nevertheless, studies focused on effectiveness of therapy and variability of diagnostic tests are scarce. ⋯ The variability of diagnostic tests used in patients with CAP is remarkable but their yield is of little effect. Patients with CAP candidates for ambulatory therapy can receive empiric antibiotic therapy with roxithromycin. In this series, roxithromycin had a high clinical effectiveness and was well tolerated.

The spontaneous hematoma of rectus abdominis sheath is a rare condition which usually presents as acute abdomen. We report our experience with 19 patients, most of them on treatment with oral anticoagulants. ⋯ Echography and computerized tomography, were useful for diagnosis. Treatment was mainly conservative and was the mode of choice; surgery was left only for those patients who had hemodynamic changes or infection of the hematoma.

[Echography-guided percutaneous biopsy of pleural lesions and effusions].
Our experience in the use of ultrasonic echography (UE) is exposed as a guide for directing small lesions and pleural effusions percutaneous lesions with an unknown grounds. We have done pleural percutaneous biopsy using UE as guide in 45 patients. The needle diameter ranges between 17 and 19.5 G. ⋯ The right result was obtained in 93% of the cases. There were not complications. We conclude that echography-directed pleural biopsy presents an excellent diagnostic profitability, it improves the results obtained with blind biopsy with Cope's needle and it must precede thoracoscopy by means of its less aggressiveness.

[Functional deterioration secondary to hospitalization for an acute disease in the elderly. An analysis of its incidence and the associated risk factors].
To determine the incidence of functional impairment at hospital admission and after hospital discharge, and to identify those factors that could be predictors of such impairment in a cohort of elderly people with high level of independence in the basic activities of daily living (BADL). ⋯ For our population, sex, hospital stay days, functional impairment in Barthel Index at admission and Cognitive Miniexam score parameters seem to behave as independent variables that can predict functional impairment risk for BADL one month after discharge. These parameters could be useful to select groups of elderly people with impairment risk, who could benefit from specialized interventions leading to prevent/decrease functional impairment secondary to acute disease and hospitalization.
