Adjustable-length loop cortical button versus interference screw fixation in quadriceps tendon anterior cruciate ligament reconstruction - A biomechanical in vitro study.
This biomechanical cadaveric in vitro study aimed to evaluate and compare the dynamic elongation behavior and ultimate failure strength of tibial adjustable-length loop cortical button versus interference screw fixation in quadriceps tendon-based anterior cruciate ligament reconstruction. ⋯ Anterior cruciate ligament reconstruction for quadriceps tendon graft using a tibial adjustable-length loop cortical button provides for comparable dynamic stabilization of the knee with increased ultimate failure load at decreased stiffness compared to screw fixation.

Randomized Controlled Trial Multicenter Study Observational StudyEffects of supplemental heat therapy in multimodal treated chronic low back pain patients on strength and flexibility.
The beneficial effects of thermotherapy on analgesia and relaxation are widely known for various diseases. To date, however, thermotherapy in chronic low back pain is not explicitly recommended in international guidelines. The effects of thermotherapy on biomechanical parameters within a multimodal back pain treatment concept are also unknown. ⋯ The implementation of thermotherapy for several hours a day (heat wrap therapy) in daily clinical practice additional to an individualized, evidence-based multimodal treatment concept can be recommended to enhance strength parameters. The potential causes of improved strength parameters as well as the meaning for the patients in activity of daily living are discussed.

Bioabsorbable plating in the treatment of pediatric clavicle fractures: A biomechanical and clinical analysis.
Classic implants for operative intervention of mid-shaft clavicle fractures in adolescents can become symptomatic, requiring removal. This study compares 1) biomechanical properties and 2) clinical outcomes in adolescents of mid-shaft clavicle fracture fixation with bioabsorbable versus metal implants. ⋯ Bioabsorbable implants had lower torsional stiffness and cantilever failure load, but comparable compression stiffness to metal implants. Bioabsorbable implants failed via gradual bending versus the catastrophic failure seen in metal implants. The clinical review allows understanding of the sequelae of this lower failure load of bioabsorbable plates where their use allowed in fracture displacement, yet achievement of ultimate radiographic union and acceptable functional outcomes.

3D correction over 2years with anterior vertebral body growth modulation: A finite element analysis of screw positioning, cable tensioning and postoperative functional activities.
Anterior vertebral body growth modulation is a fusionless instrumentation to correct scoliosis using growth modulation. The objective was to biomechanically assess effects of cable tensioning, screw positioning and post-operative position on tridimensional correction. ⋯ This comparative numerical study showed the biomechanical possibility to adjust the fusionless instrumentation parameters to improve correction in frontal and sagittal planes, but not in the transverse plane. The convex side stresses increase in the supine position may suggest that growth modulation could be accentuated during nighttime.

The effect of ankle distraction on arthroscopic evaluation of syndesmotic instability: A cadaveric study.
To assist with visualization, orthopaedic surgeons often apply ankle distraction during arthroscopic procedures. The study aimed to investigate whether ankle distraction suppresses fibular motion in cadaveric specimens with an unstable syndesmotic injury. ⋯ Since arthroscopic findings of syndesmotic instability are subtle the differential values of the syndesmotic measurements taken on and off distraction are clinically relevant. To optimally assess syndesmotic instability one should evaluate the syndesmosis without distraction or focus on fibular motion in the sagittal plane when distraction is required.
