Review Case Reports[Tracheobronchomegaly: an exceptional predisposing factor for pulmonary aspergillomas and massive hemoptysis].
Mounier-Kuhn syndrome--or tracheobronchomegaly--is a rare congenital disorder characterized by significant dilation of the trachea and main bronchi. It is accompanied by ineffective cough and is often complicated by recurrent lung infections and bronchiectasis. Clinical presentation varies widely, ranging from forms with scarce involvement of functional capacity to others that progress to respiratory failure that can prove fatal. We report an exceptional case of massive hemoptysis secondary to the presence of pulmonary aspergillomas and bilateral bronchiectasis in which bronchial arteriography with embolization and surgical resection were insufficient for resolving the complications.

Comparative Study[A comparison of several measurement scales for assessing dyspnea in their daily activities in patients with chronic obstructive lung disease].
The objective of this study was to compare two instruments for measuring dyspnea to functional capacity, defined as maximal oxygen uptake in a stress test limited by symptoms. We carried out a descriptive study to determine correlation between the methods. Subjects with severe chronic obstructive pulmonary disease were enrolled and dyspnea during daily activities was evaluated using the Medical Research Council (MRC) scale and the chronic respiratory disease health perception questionnaire (CRQ). ⋯ We also observed that the clinical scales for dyspnea correlated with each other, as did the VAS and Borg scale, but that there was no correlation between the two types of measurement. We conclude that clinical dyspnea indexes (CRQ and MRC) correlate well with maximal aerobic capacity and with each other. However, the VAS and Borg scale do not correlate with functional capacity measured by peak VO2 and VE.

Review Case Reports[Pulmonary metastases of malignant melanoma. A rare endobronchial presentation].
Malignant melanoma has a tendency to metastasize to the lung in the course of tumor growth. Many such cases have been described in the literature, although cases of endobronchial metastasis of this type of tumor revealed during fiberoptic bronchoscopy are difficult to find. ⋯ After tissue inspection, the initial suspicion of metastasis of malignant melanoma was confirmed. We review the prevalence, radiologic presentation, prognosis and treatment options for this type of metastasis.

Comparative Study[Role of dyspnea in quality of life of the patient with chronic obstructive pulmonary disease].
To analyze the correlation between quality of life and 1) lung function parameters at rest and during exercise, and 2) mean baseline dyspnea measured on two scales--Mahler's baseline dyspnea index (BDI/TDI) and the Medical Research Council (MRC) scale. We sought to observe the factor or factors having the greatest impact on the quality of life of such patients. ⋯ Dyspnea, particularly when expressed as BDI/TDI but also as measured on the MRC scale, correlates more highly with quality of life than does any other parameter. This indicates that dyspnea has greater impact than other factor on quality of life and that BDI/TDI provides a good baseline assessment of dyspnea in COPD patients.
