A pain relief score 50% of the maximum is often used as a clinically meaningful outcome in meta-analyses of analgesic trials. This arbitrary value requires validation. ⋯ The analysis suggests that the arbitrary 50% cut-off point TOTPAR score often used in meta-analyses of analgesic trials in dental pain is reasonably acceptable. This is different to the 33% cut-off point reported for analgesic trials of acute breakthrough cancer pain and some chronic pain states such as diabetic neuropathy and postherpetic neuralgia. These differences deserve careful consideration when reading reports of analgesic trials and meta-analyses. Remedication itself should be considered as the preferred outcome measure for analgesic trials.

To evaluate the efficacy and tolerability of frovatriptan in acute migraine treatment. ⋯ The available evidence suggests that frovatriptan is more effective but may cause more adverse events than placebo in the treatment of acute moderate to severe migraine. It is effective in providing pain relief and reducing the risk of recurrence. However, its effectiveness relative to other more established agents needs to be better defined by appropriate head to head trials.

To retrospectively investigate elevation of serum potassium when spironolactone (25 or 50 mg/day) and furosemide were administered concomitantly with an angiotensin II converting enzyme inhibitor (ACE-I) or angiotensin II receptor blocker (ARB) to patients with chronic heart failure for 12 months and occurrence of hyperkalemia and hypokalemia because of concomitant administration of spironolactone plus an ACE-I or ARB and furosemide. ⋯ The occurrence of hyperkalemia in patients administered spironolactone is influenced by the dose, but when it is used concomitantly with enalapril, losartan or candesartan, the occurrence of hyperkalemia exceeding 5.5 mEq/L may increase even if the dose of spironolactone is as low as 25 mg. Thus it is essential to always monitor serum potassium.

Current concepts in the endocrine therapy of breast cancer: tamoxifen and aromatase inhibitors.
Recent results from randomized controlled trials have indicated that aromatase inhibitors have superior anticancer efficacy and toxicity profiles compared with tamoxifen in the treatment of post-menopausal women with node-negative hormone receptor positive breast cancer. This has led clinicians to question whether adjuvant tamoxifen therapy is still justified. ⋯ There are limitations to the use of these drugs, and they have side effects, which require further clarification. In addition, there are certain niche advantages to the use of tamoxifen, and this drug has undergone rigorous appraisal over the last 20 years.

To assess the appropriateness of use of antimicrobial prophylaxis in paediatric surgery in Singapore. ⋯ The results showed a significantly high level of inappropriate use of antimicrobial prophylaxis in paediatric surgery in Singapore. However, when the individual factors such as appropriate choice of antibiotics, appropriate timing and duration were considered, the situation was very similar to the results obtained from overseas studies.
