Multicenter StudyA multicentric survey of the practice of hand hygiene in haemodialysis units: factors affecting compliance.
This study intended to investigate the degree of compliance with hand hygiene and use of gloves by health workers in haemodialysis (HD) units, and the factors that influenced adherence to hand hygiene protocols. ⋯ The overall adherence of health care workers to recommended hand washing practices is low. Whether or not programmes promoting higher hand hygiene standards and the potential use of alcohol-based hand cleansers will improve hand hygiene practices in HD units requires further investigation.

Clinical features and outcome of chronic dialysis patients admitted to an intensive care unit.
Information about chronic dialysis (CD) patients admitted to intensive care units (ICU) is scant. This study sought to determine the epidemiology and outcome of CD patients in an ICU setting and to test the performance of the Simplified Acute Physiology Score (SAPS II) to predict hospital mortality in this population. ⋯ CD patients admitted to the ICU are a subgroup of patients with high mortality and SAPS II can be used to assess their probability of hospital mortality. The severity of the acute illness responsible for ICU admission and an abnormal value of serum phosphorus are determinants for ICU mortality.

Tailoring high-cut-off membranes and feasible application in sepsis-associated acute renal failure: in vitro studies.
As removal of pro-inflammatory cytokines is limited in conventional diffusive or convective extracorporeal therapies, we studied in two polysulphone membranes with an industrial albumin sieving coefficient of 0.05 (Type A) and 0.13 (Type B) cytokine (IL-6, IL-8, IL-1beta, IL-1ra, TNF-alpha) and plasma protein (albumin, cystatin C, total proteins) permeability profiles. Based on the convective membrane permeability, we evaluated in vitro the dialytic modality that could provide an acceptable balance between high cytokine and low albumin clearances. ⋯ Our studies show that a mixed convective and diffusive technique ensures high cytokine clearances with an acceptable loss of albumin.

Agonist of peroxisome proliferator-activated receptor-gamma, rosiglitazone, reduces renal injury and dysfunction in a murine sepsis model.
Agonists of the peroxisome proliferator-activated receptor-gamma may help to regulate inflammation by modulating the production of inflammatory mediators and adhesion molecules. The purpose of this study was to determine the protective effects of rosiglitazone on renal injury in a sepsis model and to explore the mechanism. ⋯ These results indicate that pre-treatment with rosiglitazone attenuated the production of TNF-alpha and IL-1beta and reduced adhesion molecule expression in renal tubular epithelial cells of LPS-treated mice. Rosiglitazone has an anti-inflammatory effect in renal tubular epithelial cells through the inhibition of NF-kappaB activation.
