Case ReportsTransthoracic echocardiographic demonstration of massive pulmonary thrombus caused by protein C deficiency.
Few cases of pulmonary embolism detected by transthoracic echocardiography (TTE) have been reported. We present a case of a patient affected by pulmonary embolism caused by protein C deficiency. ⋯ Our experience demonstrates a massive pulmonary thrombus resulting from such a deficiency. Transthoracic echocardiography should be considered as the first diagnostic method for patients with suspected pulmonary embolism.

Case ReportsTissue harmonic imaging: a new method for predicting left ventricular thrombus?
We report a case of spontaneous echocardiographic contrast in the left ventricle visualized by transthoracic second harmonic imaging, but not by fundamental imaging, and subsequent thrombus formation.

Case ReportsAn unusual case of acquired ventricular septal defect as a complication of aortic valve endocarditis: echocardiographic delineation of multiple subvalvular complications in one patient.
We describe a patient with unusual echocardiographic features of prosthetic aortic valve endocarditis. Transesophageal echocardiography in this patient demonstrated a large multiloculated aortic root abscess extending to mitral-aortic intervalvular fibrosa, a high acquired ventricular septal defect, paravalvular aortic regurgitation caused by aortic valve dehiscence, and anterior mitral valve leaflet perforation.

Case ReportsPersistent hypoxemia occurring as a complication of tricuspid valve endocarditis.
A 33-year-old woman had intravenous drug-associated tricuspid valve infective endocarditis. Despite resolution of septic pulmonary emboli, hypoxemia persisted. We report a case of right-to-left shunting across a previously insignificant patent foramen ovale documented by contrast transesophageal echocardiography. Although a rare complication of tricuspid endocarditis, clinicians should be aware of this potential correctable complication.

Case ReportsIntra-aortic balloon pump associated with dynamic left ventricular outflow tract obstruction after valve replacement for aortic stenosis.
An unstable patient with critical aortic stenosis had an intra-aortic balloon pump placed preoperatively for hemodynamic support and alleviation of symptoms. After separation from cardiopulmonary bypass following aortic valve replacement, the patient was hypotensive with increased pulmonary artery pressures. ⋯ This case report demonstrates a potentially significant untoward effect of intra-aortic balloon pump augmentation after aortic valve replacement for aortic stenosis. The timely diagnosis of this iatrogenic condition in the operating room permitted the prompt implementation of appropriate management strategies and avoided unnecessary surgical intervention.
