To report the occurrence of acute myocardial infarction (MI) associated with the intake of oral sildenafil (Viagra) in a nitrate-free patient without previous history of coronary artery disease. ⋯ This report shows a rare sildenafil-associated MI in a nitrate-free patient without a previous history of coronary artery disease.

To evaluate the extent to which oximetry, spirometry and dyspnea scoring can reflect hypoxemia and hypercapnia among patients admitted to the emergency department (ED) with acute exacerbations of chronic obstructive pulmonary disease. ⋯ SpO2 alone appears to be as highly specific as a combination of other tests in the evaluation of hypoxemia. A cutoff value for SpO2 of < or = 88.5% is proposed as a criterion in screening for hypoxemia.

This prospective study aimed to evaluate inter-observer agreement of the AO/OTA and Evans/Jensen classification systems with an emphasis on the effect of reviewer experience. In addition, the level of agreement in the determination of fracture stability across varying levels of experience was evaluated. ⋯ The AO/OTA classification should be used in favor of the Evans/Jensen classification whenever possible. Our findings suggest that surgeons' perceptions about stability vary to a significant extent thereby necessitating clear definitions of stability.

This study describes a patient with nephrotic syndrome who experienced recurrent pulmonary emboli as renal disease relapsed frequently. ⋯ The results of this study suggest that inferior vena cava filter implantation can be effective for preventing recurrent pulmonary embolism complicating refractory nephrotic syndrome.

To describe the application of lateral arm free flap (LAFF) in reconstruction of defects in the oral and maxillofacial regions following ablative oncological surgery. ⋯ For the repair of moderate-sized defects of the maxillofacial area, especially in male patients, the LAFF can be recommended.
