We investigated the effects of selected disinfecting solutions on the structural integrity of Goldmann applanation tonometer tips. Hydrogen peroxide, isopropyl alcohol, dilute sodium hypochlorite (household bleach), and glutaraldehyde-phenate (Sporicidin) have been used for disinfecting tonometer tips. However, the effect of these solutions on the tips' structure has not been fully investigated. ⋯ During the soaking period, the tips were regularly removed from solution, examined through a slitlamp, measured with vernier calipers, and photographed to monitor for structural degradation. Alcohol was the only solution that damaged the tips. The other disinfecting solutions appear to be safe for use on the tips after the equivalent of 1 working year of routine use.

A 66-year-old white male presented with complaints of redness of his left eye, blurred vision, lid edema, and an intermittent throbbing pain above his left eye for 4 days. Significant clinical findings included mild hyperemia and edema of the left upper lid, marked engorgement and tortuosity of the conjunctival and episcleral vessels, mild chemosis, and extreme limitation of the extraocular movements of the left eye in all directions of gaze, especially adduction. This eye also showed a measurable proptosis with positive retropulsion. ⋯ A CT scan revealed an enlarged medial rectus muscle, including its tendinous insertion with a normal cavernous sinus. On the basis of the clinical findings and the CT scan, a diagnosis of orbital pseudotumor was made. The patient was treated with high dose systemic prednisone, which resulted in a dramatic improvement of symptoms and signs.

We evaluated the pH of six hydrogen peroxide (H2O2) soft lens disinfection systems which had over-the-counter (OTC) H2O2 substituted for the manufacturer's recommended H2O2. Substitution of four brands of OTC H2O2 into the five two-step disinfection systems resulted in a pH after neutralization which ranged from 6.70 to 7.55 pH units. There was a small but statistically significant difference in the pH after neutralization when OTC H2O2 was substituted for the manufacturer's recommended H2O2. ⋯ The pH after neutralization for the OTC H2O2 ranged from 3.35 to 4.77 pH units. This range is below the ocular comfort range of 6.6 to 7.8 pH units. These findings, along with other possible differences between OTC H2O2 and the manufacturer's recommended H2O2, indicate that OTC H2O2 should never be substituted for the manufacturer's recommended H2O2 in any H2O2 soft lens disinfection system.

Refractive amblyopia may occur as a unilateral or bilateral condition. Although bilateral refractive amblyopia may account for 1 to 2% of all refractive amblyopia, there is little consistent information in the literature regarding isoametropic amblyopia resulting from bilateral hyperopia. Hence, this retrospective study investigated the prevalence of reduced aided acuity in patients aged 10 years and younger (mean age 3.97 years) with 5 D or more of isometropic hyperopia and considered the following factors that may influence visual acuity: (1) age at first correction; (2) magnitude of hyperopia; and (3) duration of refractive correction of the hyperopia. ⋯ The magnitude of the hyperopia appeared to have the greatest influence on the visual acuity outcome both at initial correction of refractive error and 1 year or longer after correction. Duration of correction also influenced the visual acuity outcome, but to a lesser extent than the magnitude of refractive error. In contrast, the age of first correction showed little correlation with visual acuity either at the time of first refractive correction or after a minimum of 1 year of correction.
