Randomized Controlled TrialLow-dose gabapentin as useful adjuvant to opioids for neuropathic cancer pain when combined with low-dose imipramine.
Painful neuropathic conditions of cancer pain often show little response to nonopioid and opioid analgesics but may be eased by antidepressants and anticonvulsants. Although gabapentin is effective in the treatment of neuropathic pain in patients with cancer, some patients experience intolerable side effects sufficient to warrant discontinuation. The aim of this study was to see whether low-dose gabapentin is effective in treating cancer-related neuropathic pain when combined with low-dose imipramine. ⋯ Low-dose gabapentin-antidepressant combination with opioids was effective in managing neuropathic cancer pain without severe adverse effects.

Case ReportsTakotsubo cardiomyopathy associated with nonepileptic seizure after percutaneous endoscopic lumbar discectomy under general anesthesia.
Nonepileptic seizures are rare complication after general anesthesia. Postoperative seizure-induced oxidative stress promotes acute catecholamine toxicity of the myocardium. ⋯ The patient subsequently developed takotsubo cardiomyopathy. We now clearly recognize that patients with seizure activities after general anesthesia may be a higher risk for takotsubo cardiomyopathy.

Case ReportsPituitary apoplexy during general anesthesia in beach chair position for shoulder joint arthroplasty.
Pituitary apoplexy is a rare but potentially life-threatening clinical syndrome caused by the sudden enlargement of pituitary adenoma secondary to infarction and/or hemorrhage. It may be the first presentation of previously undiagnosed pituitary adenoma. Although various precipitating factors of pituitary apoplexy are indicated, the pathogenesis remains unknown. In this report, we describe for the first time a case of pituitary apoplexy developed explicitly during general anesthesia supplemented with interscalene brachial plexus block in beach chair or barbershop position for shoulder joint arthroplasty.

Comparative StudyAnesthetic management in pediatric liver transplantation: a comparison of deceased or live donor liver transplantations.
Pediatric liver transplantations (LT) are becoming increasingly more common in the treatment of a child with end-stage liver disease. The aim of this study was to evaluate the perioperative anesthetic experience of pediatric patients undergoing deceased and live donor liver transplantations. ⋯ The results of this study show that among pediatric patients LDLT continues to become an 'obligatory' option that is associated with longer operation times and higher RBC and fluid requirements than DDLT. As a marker of successful LT, higher extubation rates immediately following surgery is achievable for both pediatric LDLT and DDLT patients.

Comparative StudyCerebral state index versus bispectral index during propofol-fentanyl-nitrous oxide anesthesia.
The aim of this study was to compare the cerebral state index (CSI) and bispectral index (BIS) during propofol-fentanyl-nitrous oxide anesthesia. ⋯ The absolute values of the BIS and CSI were not significantly different during propofol-fentanyl-nitrous oxide anesthesia. The start of the measurement was faster with the CSI than with the BIS after switch-on, and measurement was less disturbed by electric cautery with the CSI.
