Clinical TrialIdeal body weight-based remifentanil infusion is potentially insufficient for anesthetic induction in mildly obese patients.
We evaluated whether the effect of remifentanil treatment differs between normal weight (NW) patients with real body weight-based remifentanil and mildly obese (Ob) patients with ideal body weight based-remifentanil during short-term anesthetic induction. We enrolled 20 patients aged between 20 and 64 years in each group (NW group: 18.5 kg/m(2) ≤ BMI < 25 kg/m(2); Ob group: BMI ≥ 25 kg/m(2)). Tracheal intubation (TI) was performed after administration of 0.5 μg/kg/min remifentanil for 5 min, including 2 min of antecedent administration, with propofol and rocuronium. ⋯ Percent increases in SBP and HR in the Ob group were significantly higher than the corresponding values in the NW group. ESC of remifentanil at the time of TI in the NW group was higher than that in the Ob group. Remifentanil treatment with anesthetic induction based on the Japanese package insert might have insufficient effects in obese patients.

Extracorporeal membrane oxygenation for 2009 influenza A(H1N1) severe respiratory failure in Japan.
To evaluate procedures and outcomes of extracorporeal membrane oxygenation (ECMO) therapy applied to 2009 influenza A(H1N1) severe respiratory failure patients in Japan. ⋯ ECMO therapy for H1N1-related severe respiratory failure in Japan has very poor outcomes, and most patients developed adverse events. However, this result does not refute the effectiveness of ECMO. One possible cause of these poor outcomes is the lack of satisfactory equipment, therapeutic guidelines, and systems for patient transfer to central facilities.

One-lung ventilation in infants and small children: blood gas values.
We investigated one-lung ventilation (OLV) in pediatric patients under 10 kg. The feasibility of OLV using either Arndt endobronchial blocker (AEB) or mainstem intubation technique is analyzed. Arterial blood gases (ABG) monitored throughout the procedures are presented. ⋯ The use of extraluminal AEB or mainstem intubation for OLV can be successfully completed in infants weighing less than 10 kg. OLV may induce acute respiratory pathology; therefore we recommend routine intraoperative ABG monitoring for pediatric patients.

The effects of intravenous anesthetics on mouse embryonic fibroblast viability and proliferation.
The aim of this study is to evaluate the cytotoxic and antiproliferating effects of intravenous anesthetics on an mouse fibroblast in vitro cell culture system. ⋯ Our results showed that intravenous anesthetics may not affect mouse fibroblast proliferation and viability.
