Deep hypothermia with circulatory arrest is the usual method of cerebral protection during replacement of the aortic arch. It has the enormous advantage of allowing the surgical repair to be carried out in a complete bloodless field with no aortic cross-clamping. However, this method only gives the surgeon a limited period of time to carry out the aortic repair. ⋯ Thirteen nonfatal neurological complications were observed. The type of lesion, gender, age, duration of CPB, cerebral perfusion, and circulatory arrest had no influence on the neurological outcome of the patients. In our experience, antegrade selective perfusion of the brain with cold blood and moderate hypothermic central temperature constitutes the method of choice for cerebral protection during surgery of the aortic arch as it requires no prolonged CPB and does not limit the time available to perform the aortic repair.

Concomitant carotid endarterectomy and coronary bypass surgery: should cardiopulmonary bypass be used for the carotid procedure?
With the increasing age of patients undergoing coronary artery bypass grafting (CABG), a greater number have associated clinically significant carotid disease. This study determined the morbidity and mortality for combined carotid endarterectomy (CEA)/CABG using cardiopulmonary bypass (CPB) for both procedures versus a combined approach using CPB only during CABG. ⋯ We conclude that combined CEA/CABG using CPB only during the myocardial revascularization procedure remains the technique of choice in patients with coronary and carotid artery disease, offering better outcome in terms of perioperative morbidity than a combined CEA/CABG using CPB for both procedures.

Minimally invasive approach for redo mitral valve surgery: a true benefit for the patient.
Redo mitral valve surgery via sternotomy is associated with a substantial morbidity and mortality. This study evaluated a minimally invasive technique for mitral valve redo procedures. ⋯ Redo mitral valve surgery can be performed safely using a minimally invasive approach in patients with a previous sternotomy. The right lateral minithoracotomy offers excellent exposure. It minimizes the need for cardiac dissection, and thus, the risk for injury. Avoiding a resternotomy increases patient comfort of redo mitral valve surgery.

Resternotomy is a common part of cardiac surgical practice. Associated with resternotomy are the risks of cardiac injury and catastrophic hemorrhage and the subsequent elevated morbidity and mortality in the operating room or during the postoperative period. The technique of direct vision resternotomy is safe and has fewer, if any, serious cardiac injuries. The technique, the reduced need for groin cannulation and the overall low operative mortality and morbidity are the focus of this restrospective analysis. ⋯ The technique of direct vision resternotomy has been associated with zero moderate or major cardiac injury/catastrophic hemorrhage at reoperation. Few patients have required groin cannulation. In the postoperative period, there was acceptable blood loss, transfusion rates, reduced morbidity, and moderate low mortality for this potentially high risk group.
