Case Reports[Ventilation during tracheotomy in extensive, 90% laryngeal stenosis using superimposed high frequency jet ventilation via the jet laryngoscope].
In a 35-year old male patient with laryngeal carcinoma an acute respiratory insufficiency with early hypoxaemia developed due to massive laryngeal stenosis. An endotracheal intubation was not possible since the available lumen was too small. Tracheotomy using local anaesthesia was not possible since spontaneous respiration with a Venturi mask applying 100% oxygen was not sufficient and the patient was becoming restless and agitated due to the hypoxaemia. ⋯ The tracheotomy was performed using the superimposed high frequency jet ventilation. Throughout the procedure oxygenation and ventilation were adequate. The laryngectomy performed several days later revealed a cauliflower type protrusion into the tracheal lumen and a 5 cm long stenosis of the larynx with a lumen of 3 mm.

[Clinical and electron microscopy study of sudden deafness treatment with the 10% HES 200/0.5 and pentoxifylline combination].
The incidence of sudden hearing loss has increased. The pathogenetic mechanisms are still unknown, but viral infections and vascular phenomena with acute impairment of microvascular perfusion are thought to play a major role. Infusion of hydroxyethyl starch (HES) is used as a regimen to treat sudden hearing loss. ⋯ Accordingly, classic antihistaminic drugs had no therapeutic effect in our patients. Dextran is used as an alternative to hydroxyethyl starch. In contrast to HES, the often mentioned higher incidence of severe anaphylactic reactions due to dextran has dramatically decreased with hapten inhibition (after preinjection of monovalent haptendextran Promit).(ABSTRACT TRUNCATED AT 250 WORDS)

Review Case Reports[Adenoid cystic carcinoma of the trachea. Case report and review of the literature on malignant tracheal tumors].
Primary tumours of the trachea are rare. In most cases one finds squamous cell carcinoma or adenoid cystic carcinoma. At the Department of Otolaryngology, Head and Neck Surgery at the Christian-Albrechts-Universität of Kiel only 19 malignant tumours of the trachea were diagnosed between 1949 and 1992 among a total of 6000 malignancies. ⋯ Of these patients we report on the case of a 57-year old female patient who was diagnosed with an adenoid cystic carcinoma of the trachea and is free of disease eight years after open resection. Since initial symptoms are nonspecific emphasis is on the diagnostic process. The latest publications on therapeutic measures are reported.

[Surgical treatment of pediatric laryngeal stenosis in the area of the ring cartilage].
In the past 15 years, infants and children with laryngeal stenosis have also been successfully treated with laryngotracheoplasty. From 1991 to 1993 we performed a surgical treatment in 20 pediatric patients with subglottic stenosis. ⋯ In 14 patients with a tracheostoma we used a modification of Réthi's division of the posterior lamina of the cricoid by the placement of a autogenous costal cartilage graft. A 6-week-period of stenting is required for sufficient healing and stability of the reconstructed airway.
