During the last three years, 79 adults suffering from acute epiglottitis have been treated in the ENT departments of the university hospital Rudolf Virchow, Berlin, 36 women (41 years of age as an average) and 43 men (average age 39 years). Acute epiglottitis developed either all of a sudden, within hours, or gradually, within days. ⋯ The inflammation responded satisfactorily to conservative antibiotic management (broad spectrum penicillin). Only one patient had to undergo intubation, none of the adults required tracheotomy.

From 1989 to 1991, 126 tonsils were prospectively studied in cadavers, volunteers and patients by B-scan ultrasonography. The transducer was positioned just medial of the mandibular angle. The submandibular gland, M. digastricus, M. stylohyoideus and the tongue proved to be suitable landmarks. ⋯ Peritonsillar abscess could not be sonographically defined in 13/15 cases. Squamous cell carcinoma presented with random echos and indistinct borders. Infiltration of the base of the tongue could be safely confirmed.

Comparative Study[High-caloric nutrition in radiochemotherapy, nasogastric tube and parenteral nutrition versus PEG (percutaneous endoscopic controlled gastrostomy)].
The advantages and problems of different types of artificial feeding are discussed. For patients needing long-term nutritional support, we see the following special advantages of PEG: physiological enteral nutrition with an only small risk of tube dislocation, no additional handicap of the natural swallowing procedure and without stigmatizing the patient by an external visible tube (Tab. 3). To help to spread this method of artificial feeding for the benefit of the patient, this article gives some informations on how to cope with the problems that might occur when using PEG (for example: a diet plan for building up the tube food nutrition (Tab. 4).

[Video fluoroscopy of the swallowing act following partial supraglottic laryngectomy].
Videofluoroscopy of swallowing was performed in 21 patients who had undergone partial horizontal laryngectomy. Food residues were present in most of these patients. Residues of liquid contrast were observed in 85.7% of the patients and residues of consistent contrast in 61.5% of the patients. ⋯ Impaired clearing of food residues results in aspiration. Aspiration of liquid contrast was observed in 76.2% of the patients and aspiration of consistent contrast in 19.1% of the patients (p less than 0.001). Aspiration after swallowing was observed in most of the patients.

[The value of ultrasound-controlled fine-needle biopsy in the diagnosis of possible neck tumors].
From October 1985 to July 1988 we performed a study to examine 106 patients with head and neck tumours, by using ultrasound as a guiding system for fine-needle aspiration biopsy. It could be shown that this method has a high diagnostic significance at the neck with a low rate of risks. In a first step puncture was effected to obtain material for cytology, if necessary also for a bacteriological examination. ⋯ In cases of benign neck cysts, neck abscesses and non-specific lymphadenopathy, a cutting neck biopsy is not required, provided the clinical diagnosis is in accordance with the result of aspiration cytology and the further clinical progress. The advantage of the ultrasound-guided puncture compared with palpation-guided puncture is the certainty of locating the region of interest even in deep lesions without an appreciable risk of complications. In our opinion, ultrasound-guided fine needle aspiration biopsy is indicated in all cases of unclear head and neck tumours which could be treated conservatively if the result of the puncture is non-malignant.
