Multicenter StudyTranexamic acid and post-tonsillectomy hemorrhage: propensity score and instrumental variable analyses.
Although post-tonsillectomy hemorrhage occurs rarely, it can be life-threatening. Previous studies showed that tranexamic acid (TXA) had insignificant association with the rate of post-tonsillectomy hemorrhage, but those findings were limited by small sample sizes. The purpose of this study was to examine the effectiveness of TXA in preventing post-tonsillectomy hemorrhage using nationwide database. ⋯ Administration of TXA from the day of tonsillectomy is not associated with reduction in reoperation or blood transfusion rates.

Novel three-dimensional image system for endoscopic ear surgery.
The objective of this study was to evaluate the clinical outcomes of conventional two-dimensional (2D) endoscope with a novel computer-based three-dimensional (3D) imaging system for otologic surgical procedures. ⋯ Our study demonstrated that the computer-based 3D imaging system enables the application of 3D vision technology to otologic surgery. The system has no obvious side effects, such as visual fatigue or time delay. It not only facilitates performing the related surgical procedures but also helps in teaching and learning endoscopic ear surgeries.

The course of lower cranial nerves within the neck: a cadaveric dissection study.
To evaluate the course of lower cranial nerves (CNs) within the neck in relation to surrounding structures and anatomic landmarks via a cadaveric dissection study. ⋯ In conclusion, our findings indicate that anatomic variations are not rare in the course of lower CNs within the neck in relation to adjacent structures, and awareness of these variations together with knowledge of distance to certain anatomic landmarks may help the surgeon to identify lower CNs during neck surgery and prevent potential nerve injuries.

Spontaneous nasal cerebrospinal fluid leaks: management of 24 patients over 11 years.
Most cases of non-traumatic nasal cerebrospinal fluid (CSF) leaks occur spontaneously without any obvious reason. Severe and life-threatening complications are possible consequences. Endoscopic repair is considered the gold standard; however, diagnosis and therapy of these CSF leaks stay challenging. ⋯ Spontaneous nasal CSF leaks often initially present with symptomatic meningitis. Imaging does not always clearly identify the skull base defect. Common localizations are the anterior ethmoid roof and the lateral sphenoid sinus. Obesity seems to be a predisposing factor. In most cases, endoscopic repair with low morbidity is possible; however, an individualized approach is necessary.
