Comparative StudyOptimal site for throat culture: tonsillar surface versus posterior pharyngeal wall.
To determine the optimal site of throat culture for the detection of potential pathogens by comparing culture results from the tonsillar surface and the posterior pharyngeal wall in children selected for adenotonsillectomy and in children without upper respiratory disease. Cotton culture swabs were taken from the tonsillar surface and the posterior pharyngeal wall of 50 children selected for adenotonsillectomy for symptoms of recurrent tonsillitis and/or adenotonsillar hypertrophy and of 50 children without upper respiratory disease. Potential respiratory pathogens were identified. ⋯ Group A beta-haemolytic streptococci were found in the samples of the posterior pharyngeal wall and the tonsillar surface in 17 and 13%, respectively (P = 0.2). When dealing with patients with sore throat, sampling both tonsillar surfaces is enough for the detection of group A beta-haemolytic streptococci. When detection of other bacteria is also important, such as for research purposes, the posterior pharyngeal wall should be sampled as well.

Case ReportsBilateral and unilateral internal carotid artery dissection causing isolated hypoglossal nerve palsy: a case report and review of the literature.
Two cases of isolated hypoglossal nerve palsy due to internal carotid artery dissection are reported. In one case, a bilateral dissection was encountered. The clinical presentation and multidisciplinary management are discussed and the literature reviewed.

Case ReportsA case of metastatic hepatocellular carcinoma in the nasal cavity.
Metastatic hepatocellular carcinoma in the nasal cavity and paranasal sinuses is rare. We report the case of a 71-year-old male afflicted with hepatocellular carcinoma with metastasis in the maxillary sinus and nasal cavity. He underwent radiation therapy with a total dose of 36 Gy, but he died of terminal liver failure. The possible metastatic route and prognosis of metastatic hepatocellular carcinoma in the sinonasal tract are discussed.

Case ReportsSinonasal fibro-osseous hamartoma: case presentation and differential diagnosis with other fibro-osseous lesions involving the paranasal sinuses.
We present the case of a 2-year-old girl who was referred to the ENT Department of the Antwerp University with a bony obstruction of the left side of the nose. The lesion originated from the ethmoid roof and the middle turbinate and extended into the nasopharynx. ⋯ A combination of clinical, radiographic and histological information permitted a diagnosis of a benign fibro-osseous lesion: sinonasal hamartoma. The benign nature of this tumor justified a conservative follow-up.

Endothelial nitric oxide synthase upregulation in the cochlea of the guinea pig after intratympanic gentamicin injection.
Single-shot transtympanic gentamicin therapy has become a popular treatment modality for Meniere's disease despite the known possible ototoxic properties of this drug. It was shown recently that NO production and iNOS were upregulated after gentamicin application, which was interpreted as a possible effect of ototoxicity. In this study we analyzed the expression of eNOS after gentamicin application to determine a possible correlation of this enzyme with gentamicin-induced ototoxicity. ⋯ Statistically significant upregulation of eNOS was observed in all cell types analyzed. Depending on the various cell types the amount of gold-labeled eNOS antibodies was 2.5 to 5.7 times higher after gentamicin application. We observed significant eNOS upregulation after gentamicin application in the cochlea, in conjunction with cellular damages and decreased hearing.
