Trabecular titanium cups in acetabular revision arthroplasty: analysis of 10-year survivorship, restoration of center of rotation and osteointegration.
In case of severe bone loss, acetabular revision can be challenged using extra porous pure trabecular titanium (TT) revision cups designed to ensure enhanced iliac and ischiatic purchase. Aim of the study is to report on the clinical and radiological results of a TT acetabular component, evaluating functional outcome, restoration of the hip center of rotation and osteointegration. ⋯ In case of severe bone loss, TT revision cup system allows for good restoration of center of rotation and osteointegration showing good 10-year survival rate.

Lumbar spinal fusion does not increase early dislocation risk in primary total hip arthroplasty through the direct anterior approach.
Patients with total hip arthroplasty (THA) and a concomitant lumbar spinal fusion (LSF) might have an increased incidence of revision surgery and postoperative complications such as early THA dislocation. The direct anterior approach (DAA) has gained popularity in THA due to its soft tissue-preserving nature and the relatively low dislocation risk. The purpose of the present study was to examine whether LSF patients undergoing minimally invasive THA through the DAA might have an increased risk of prosthetic-related complications compared to matched-control patients without a LSF. ⋯ Retrospective case-control study, III.

Recreational activity after cementless total hip arthroplasty in patients older than 75 years.
This retrospective study aimed to compare activity levels before and at mid-term follow-up after primary cementless total hip arthroplasty (THA) in patients older than 75 years. ⋯ The majority of patients maintained a recreational activity level in the mid-term after primary cementless THA. However, a change in disciplines toward low-impact activities was observed.

Primary total knee replacement for tibial plateau fractures in older patients: a systematic review of 197 patients.
Tibial plateau fractures are typically treated with osteosynthesis. In older patients, osteosynthesis is associated with some complications, risk of post-traumatic osteoarthritis and long partial, or non-weight bearing during the recovery phase. To avoid these problems, primary total knee replacement (TKR) has become an increasingly common treatment option. The aim of this study was to evaluate all the relevant literature and summarize the current evidence-based knowledge on the treatment of tibial plateau fractures with primary TKR in older patients. ⋯ Based on low-quality evidence, TKR appears to be a useful treatment option for tibial plateau fractures in older patients. Controlled trials are mandatory to determine the relative superiority of these two options as primary treatment of tibial plateau fractures in older patients.

Use of contralateral lateral meniscus for medial meniscal allograft transplantation: a cadaveric study.
Meniscal allografts are a preferred alternative to menisectomy in cases of irrepairable meniscal tears in young patients. Biological meniscal allograft transplantation requires a cadaveric donor, limiting its availability for transplantation. We are exploring the possibility of using contralateral lateral meniscus for medial meniscal allograft transplantation, as it can be easily procured from proximal tibial cuts from total knee replacement. ⋯ Diagnostic study.
