Total elbow arthroplasty in patients who have elbow fractures caused by gunshot injuries: 8- to 12-year follow-up study.
In this retrospective study, we aimed to present 8-12-year clinical and radiographic evaluation of total elbow arthroplasty in young patients who had open fractures due to gunshot injuries. ⋯ As a result, the patients in whom we implemented total elbow prosthesis in comminuted elbow fractures due to gunshot wounds seemed to get back into active life in the early period without any problem and it seemed that their pain disappeared and their functional capacity increased. In the long period, however, these values showed a distinct decrease with the same patients.

Drilling K-wires, what about the osteocytes? An experimental study in rabbits.
The function of osteocytes regarding osteonecrosis has been underestimated for a long time. Recently it has been suggested that apoptosis of osteocytes results in strong osteoclastic bone resorption. Death of osteocytes due to drilling may therefore increase the risk of K-wire loosening. The purposes of our in vivo study were to assess the minimal drill time needed to notice disappearance of osteocytes and to measure the distance of the empty osteocyte lacunae surrounding the drill tract in relation with the insertion time, directly and 4 weeks after drilling Kirschner (K-) wires into the femur and tibia of rabbits. ⋯ Although only drilling without cooling was studied, short drilling times may prevent the disappearance of osteocytes in case cooling is not used in clinical practice as is the case in percutaneous K-wire insertion.

Randomized Controlled TrialSimplified wound care and earlier wound recovery without closed suction drainage in elective total hip arthroplasty. A prospective randomized trial in 100 operations.
Evidence to support or refute closed suction drainage (CSD) in primary total hip replacement (THR) is not conclusive. Our anecdotical experience was that persistent ooze from the drainage hole often delayed wound recovery. We hypothesized that, without CSD, wound care would be simplified without short or long term disadvantage. ⋯ To omit CSD in primary THR results in simplified and more rapid wound management without any disadvantage at short and long term.

Case ReportsInferior dislocation of the proximal tibiofibular joint: a new type of dislocation with poor prognosis.
Dislocation of the proximal tibiofibular joint is an unusual injury. We report a patient, who developed inferior proximal tibiofibular dislocation after a severe motorcycle accident. ⋯ Classifications of proximal tibiofibular dislocations did not include inferior dislocation. This type is always associated with avulsion mechanism and has the poorest prognosis.

Dislocation of the elbow with fractures of the coronoid process and radial head.
The aim of the study was to evaluate the factors relevant to prognosis after operative treatment of an elbow dislocation/fracture involving the coronoid process and the radial head. In 30-50% of cases, elbow dislocations are accompanied by concomitant bony injuries. Here, the ulnar coronoid process and the radial head are particularly crucial to the stability of the elbow joint. ⋯ Elbow dislocations with involvement of the ulnar coronoid process and the radial head are complex injuries and their surgical treatment and aftercare need to be handled by a skilled and experienced traumatologist. In this process, the precondition for regaining a stable joint with good function is, above all, early, exercise-stable fixation and/or reconstruction of the coronoid process and early functional mobilization of the joint.
