ReviewLabor analgesia for the parturient with cardiac disease: what does an obstetrician need to know?
Maternal heart disease complicates 0.2-3% of pregnancies. The optimal management of the pregnant patient with cardiac disease depends on the cooperative efforts of the obstetrician, the cardiologist and the anesthesiologist involved in peripartum care. A comprehensive understanding of physiology of pregnancy and pathophysiology of underlying cardiac disease is of primary importance in provision of obstetric analgesia or anesthesia for this high-risk group of patients. This article will review the current guidelines and standards pertinent to management of obstetric analgesia and anesthesia in parturients with cardiac disease.

Comparative StudyDoes pain relief during delivery decrease the risk of postnatal depression?
To test the hypothesis that sufficient pain relief during delivery decreases the risk of postnatal depression. ⋯ The mode of pain relief during vaginal delivery seems to be associated with the incidence of postpartum depression, especially immediately after delivery.

Intrapartum influences on cesarean delivery in multiple gestation.
To evaluate which intrapartum factors influence the method of delivery in a group of twin pregnancies eligible for vaginal delivery. ⋯ Twin gestations with twin B presenting as vertex or breech or with an epidural were less likely to undergo combined vaginal delivery and cesarean section. The liberal use of epidural anesthesia may reduce the need for cesarean section in patients with twin gestations considered good candidates for vaginal delivery.

Randomized Controlled Trial Clinical TrialPreoperative transcervical analgesia for laparoscopic sterilization with Filshie clips. A double blind randomized trial.
To evaluate the use of preoperative transcervical analgesia for reducing postoperative pain from laparoscopic sterilization with Filshie clips. ⋯ Preoperative transcervical perfusion of the uterus, fallopian tubes and into the pelvis did not reduce postoperative pain or request for analgesics after laparoscopic sterilization with Filshie clips.

Multivariate analysis of histopathologic prognostic factors for invasive cervical cancer treated with radical hysterectomy and systematic retroperitoneal lymphadenectomy.
The aim of this study was to identify the independent histopathologic prognostic factors for patients with cervical carcinoma treated with radical hysterectomy including paraaortic lymphadenectomy. ⋯ LN status, parametrial invasion, LVSI, and histology of pure adenocarcinoma are important histopathologic prognostic factors of cervical carcinoma treated with radical hysterectomy and systematic retroperitoneal lymphadenectomy. Prognosis for patients with cervical carcinoma may be stratified by combined analysis of these histopathologic prognostic factors. Postoperative therapy needs to be individualized according to these prognostic factors and validated for its efficacy using randomized clinical trials.
