Intra-arterial thrombolysis may reduce mortality in acute basilar artery (BA) occlusion. We wanted to identify an easy-to-use pre-treatment MR-based predictor of clinical outcome in patients with acute BA occlusion treated with IAT. ⋯ In patients with acute BA occlusion, treated with intra-arterial recombinant tissue plasminogen activator, a high number of acute ischaemic lesions, based on diffusion-weighted imaging, is a predictor of poor clinical outcome.

To test whether portable duplex ultrasound devices can be used in a prehospital '911' emergency situation to assess intracranial arteries. ⋯ Emergency assessment of intracranial arteries using portable duplex ultrasound devices is feasible shortly after arrival at the patient's site.

Brain natriuretic peptide as a surrogate marker for cardioembolic stroke with paroxysmal atrial fibrillation.
Cardioembolic stroke generally results in severer disability, since it typically has a larger ischemic area than the other types of ischemic stroke. However, it is difficult to differentiate cardioembolic from noncardioembolic stroke (atherothrombotic and lacunar stroke), whenever ischemic stroke patients have sinus rhythm at the time of presentation. ⋯ From our results, it was suggested that the first-day brain natriuretic peptide and left atrial appendage flow measurements would be helpful in differentiating cardioembolic stroke with atrial fibrillation from noncardioembolic stroke.

Clinical features associated with early hospital arrival after acute intracerebral hemorrhage: challenges for new trials.
Early hospital admission followed by correct diagnosis with minimum delay is a prerequisite for successful new interventions in acute intracerebral hemorrhage (ICH). The aim of this study was to evaluate clinical features associated with early hospital arrival in ICH patients and their influence on the outcome. ⋯ Treatment decisions in acute ICH may need to establish new combined approaches to maximize the number of eligible patients for early therapy considering the interactions between independent outcome predictors presented at early onset.

Case fatality of stroke and day of the week: is the weekend effect an artifact? Takashima stroke registry, Japan (1988-2003).
For stroke admissions, the 'weekend effect' has been associated with higher stroke fatality. However, it is unclear if stroke case fatality shows this pattern if the onset day is taken into account. Day of the week variation in stroke case fatality was examined using 16-year stroke registration data. ⋯ Stroke fatality rates based on the day of admission were higher during the weekend than weekdays, although the difference did not reach statistical significance. However, this trend disappeared when the fatality rate was based on the day of onset.
