Comparative Study Clinical TrialIs there any complimentary role of F-18 NaF PET/CT in detecting of osseous involvement of multiple myeloma? A comparative study for F-18 FDG PET/CT and F-18 FDG NaF PET/CT.
Multiple myeloma (MM) is a disease characterized by a monoclonal plasma cell population in the bone marrow whereby osseous involvement is a predominant feature. The aim of this prospective study was to investigate the combined use of F-18 FDG and F-18 NaF PET/CT in the skeletal assessment of patients with MM and to compare the efficacy of these two PET tracers regarding detection of myeloma-indicative osseous lesions. A total of 26 patients (14 females and 12 males, mean age 61.8 ± 1.8 years (range 40-81 years)) with MM diagnosed according to standard criteria. ⋯ On the other hand, F-18 NaF PET/CT demonstrated additional 135 bone lesions defined as rib fractures and other findings due to degenerative changes. In conclusion, our study implies that F-18 NaF PET/CT scan did not actually aid for assessing the myelomatous bone lesions in patients with MM. Therefore, a complementary F-18 NaF PET/CT may be an accurate modality for detecting of bone fracture in patients with MM.

Association between early peak temperature and mortality in neutropenic sepsis.
Fever is often the first sign of neutropenic infection, but its prognostic impact has not been established. We aimed to determine whether early peak temperature is associated with mortality in patients with neutropenic sepsis admitted to intensive care units (ICUs). We used a database of admissions to 157 ICUs in Australia and New Zealand between 2005 and 2013 to seek an association between peak temperature within the first 24 h in ICU and in-hospital mortality in neutropenic and non-neutropenic sepsis. ⋯ In neutropenic sepsis patients admitted to ICU, a temperature below 36.5 °C is associated with increased mortality compared with normothermia. In contrast to non-neutropenic sepsis, fever was not associated with a significant reduction in mortality in neutropenic patients. Interventional studies are needed to determine whether physical or pharmacological measures to reduce fever influence outcomes during neutropenic infections.

Review Comparative StudyCauses of resistance and treatment choices of second- and third-line treatment in chronic myelogenous leukemia patients.
For patients with chronic myelogenous leukemia who fail first-line therapy, several factors should be considered for the decision of the next treatment option. Second-generation tyrosine kinase inhibitors (TKIs) dasatinib, nilotinib, and bosutinib offer improved potency and a high likelihood of success for these patients. Overall, efficacy data are comparable for these agents, and so physicians should consider the BCR-ABL1 mutation profile and the patient's history to make a decision on the best choice. ⋯ However, optimal dose of ponatinib is still under investigation. Overall, it is recommended to select a drug that minimizes the likelihood of worsening the patient's past side effects or comorbid conditions. In any case, chance and risk of allogeneic stem cell transplantation should be compared with the long-term outcome of TKI therapy in patients eligible for this procedure.
