Review[Acute renale failure in burns - causes, consequences and avoidance strategies].
Acute renal failure (ARF) is a common and dangerous complication in intensive care medicine. Especially critical ill patients, who are suffering from major burns, have a high risk to develop ARF as a consequence of their trauma. ⋯ These circumstances have a major impact on the morbidity and mortality of severely burned patients. The aim of this manuscript is to review the reasons for the development of an ARF in burn patients as well as its consequences; moreover it highlights potential strategies to avoid ARF in critically ill burned patients.

Review[Point-of-care-monitoring of coagulation].
In recent years, point-of-care coagulation monitoring systems have gained widespread use during surgical procedures. By providing quick and goal directed diagnostic information, they enable the attending physician to deliver a quick and goal directed therapy, thereby saving blood products and reduce resource utilisation. ⋯ Indications and limitations are described with special emphasis on the legal prerequisites that have been changed according to the new guidelines for laboratory investigations launched by the German college of physicians in 2008. Even though several issues regarding the necessary quality controls are still under debate especially with the more complex methods, it is of paramount importance that a comprehensive quality management is established for these POC methods.

[Update Reanimation 2010: Main changes of the new resuscitation guidelines].
Daily routine procedures in anesthesia may be associated with critical events including cardiac arrest. Therefore every anesthesiologist must be informed completely about standards and guidelines for resuscitation to provide adequate patient safety. In October 18 (th), 2010, new international guidelines for resuscitation were published. The main changes in these resuscitation guidelines are described.

Review[Presence and future of emergency medicine in Germany].
Health care policy has changed duties and responsibilities of hospitals in Germany. The transition zone of in- and outpatient care has been recognized as a critical gateway for the success of hospitals, subsequently leading to the appreciation of the value of professionalized emergency departments. ⋯ Furthermore, those strategies are accompanied with the improvement of economic characteristics. Summing up, professionalisation of hospital-based emergency medicine in Germany is urgently needed and may improve medical and economic success of hospital-based patient care.
