Impaired mental function, from clouding of consciousness to deep coma is often seen in patients with systemic inflammation. Diagnosis of this syndrome which is called "septic encephalopathy" is dependent on exclusion of other causes. The underlying mechanisms have only been defined in parts. ⋯ Further effects of the cytokine expression are perivascular edema and hemorrhage. The loss of metabolic regulation of the brain perfusion and local cerebral ischemia additionally contribute to the etiology of septic encephalopathy. A specific therapy is not yet known.

[Contribution of german university departments of anesthesiology to the international literature].
According to the president of the "Wissenschaftstrat", Prof. W. Schulze, the university is in the center of research and scientific development. The contributions of the different anesthesia departments of all german universities in international anesthesia and intensive care medicine journals were studied in a descriptive analysis. ⋯ Marked differences in the numbers of publications of the different universities in anesthesia and intensive care medicine journals were observed. Universities from former East Germany are still less involved in international publications.

Case Reports[Horizontal deceleration trauma with diffuse decollement bleeding--a casuistry].
Treatment of severe haemorrhage caused by multiple trauma is a serious challenge to preclinical as well as clinical management. This is a case report of a motorcycle accident in which a patient sustained total amputation of both legs. Following adequate preclinical care, vital indication led to the patient's immediate surgical treatment. ⋯ Due to its extent, the bleeding could not be treated surgically, nor did it allow of haemodynamic stabilisation despite continuous massive transfusion. Retrospectively, the impressing amputation injury was treated successfully. In spite of all available surgical and intensive care efforts, however, the slowly demasking monstrous decollement with diffuse tissue bleeding proved to be an injury pattern leading to the patient's death.
