Efforts to improve patient safety require an understanding of organizational culture. In a survey of inpatient healthcare providers in a children's hospital, physician perceptions of teamwork were higher than those of all other staff (P < .001). ⋯ A majority of respondents did not feel rewarded for incident reporting. Information on hospital-level safety culture can lead to targeted system improvement.

The purpose of this article is to share authors' experiences associated with the Transforming Care at the Bedside initiative, a national program funded by the Robert Woods Johnson Foundation with direction and technical assistance provided by the Institute for Healthcare Improvement. The article discusses the innovation process and the change cycles, impact of various innovations, and lessons learned.

The use of evidence-based practice for improving patient care outcomes is discussed extensively in the literature. This article describes how a model for change to evidence-based practice developed by Rosswurm and Larrabee was used for the effective implementation of the American Stroke Association's Acute Stroke Program in an acute care hospital. The results demonstrated that patient care and outcomes were improved through the implementation of this exemplar of evidence-based practice.

How satisfied are parents supported by nurses with the NIDCAP model of care for their preterm infant?
The main purpose of implementing the Newborn Individualized Developmental Care and Assessment Program (NIDCAP) in our neonatal intensive care unit from the perspective of quality of care was to bring about an improvement in the satisfaction of parents. This was measured by means of the NICU-Parent Satisfaction Form and the Nurse Parent Support Tool. Parents were significantly more satisfied with care given according to NIDCAP principles than they were with the traditional care for their premature born babies.

This article describes the application of the Iowa Model of Evidence-Based Practice to Promote Quality Care to creating standards of practice for Basic Cardiac Life Support certification of the nursing staff at a merged health center in Montreal, Quebec, Canada. Selected outcomes and adopted practice guidelines are presented.
