The role of antibiotic prophylaxis in totally implantable venous access device placement: results of a single-center prospective randomized trial.
This study evaluated whether prophylactic treatment with a cefazolin could prevent infections in patients who had a surgically inserted totally implantable venous access device (TIVAD). ⋯ We do not recommend the use of prophylactic antibiotics in TIVAD insertion because they will not decrease the already low rate of postoperative infectious complications. The nonoperative management (NOM) of abdominal gunshot injuries is gradually becoming the standard of care. Patients with pelvic gunshot injuries constitute a subgroup of patients at high risk of visceral injury. The aim of this study was to assess the feasibility and safety of the selective NOM of pelvic gunshot injuries. ⋯ Selective NOM of pelvic gunshot injuries is a feasible, safe, and effective alternative to routine laparotomy.

Validation of noninvasive hemoglobin measurements using the Masimo Radical-7 SpHb Station.
Hemoglobin levels must be obtained through blood draws, which are invasive, time-consuming, and provide only 1 data point at a time rather than continuous measurements. The Masimo Radical-7 SpHb Station (Masimo Corporation, Irvine, CA) has been shown by its manufacturers to provide accurate noninvasive hemoglobin measurements in physiologically normal patients. The objective of this study was to validate noninvasive hemoglobin measurements using the Masimo Radical-7 device. ⋯ Noninvasive hemoglobin monitoring is a new technology that correlated with laboratory values and supports the continued study of noninvasive hemoglobin monitoring.

Trauma induces a hypercoagulable state that is resistant to hypothermia as measured by thrombelastogram.
The aim of this study was to test the hypothesis that severely injured trauma patients would be hypercoagulable compared with controls measured by thromboelastography and that this hypercoagulability would persist over a broad range of temperatures. ⋯ Severely injured patients are more hypercoagulable than controls throughout a broad range of temperature. Decreasing temperature has a greater effect on coagulation in controls compared with patients.
